The Community Living Counselor / Tailored Day Worker role at Home of Guiding Hands is a part-time position offering an hourly wage ranging from $18.00 to $20.00. This position is ideal for individuals passionate about making a meaningful difference in the lives of adults with intellectual and developmental disabilities. As a Community Living Counselor, you will be more than an employee; you will serve as a guide, mentor, and advocate for clients, empowering them to live as independently as possible while achieving their personal goals.

In this hands-on role, you will work closely with assigned clients to schedule, plan, and carry out visits that focus on their personalized Individual Service Plans (ISPs). Life skills training is a core component of the job, involving direct teaching and support in essential activities such as budgeting, meal preparation, accessing health services, managing household tasks, navigating government benefits, and fostering healthy relationships and personal safety. Beyond teaching, you will collaborate with clients to develop and update their goals, prepare ISP reports, and actively participate in ISP meetings. Communication and collaboration with an interdisciplinary team will be vital to delivering cohesive and effective support services.

As part of your daily duties, you will travel within San Diego or Imperial County to meet clients in their homes and community settings, requiring your use of a reliable vehicle and a valid California driver’s license. Accurate and timely documentation of client progress and program activities is essential to maintain quality care standards and meet program requirements. The role demands excellent interpersonal skills, a proactive approach, and the ability to use technology for communication and documentation tasks.
