This specific opportunity is for a Community Health Worker (CHW) position located in Cook County and surrounding areas. The CHW acts as a critical link between community members and healthcare providers, fostering stronger relationships and improving health outcomes by delivering outreach, education, and support. This full-time, field-based role requires someone who can travel extensively within a 90-mile radius of Cook County, working about 70 percent of the time in the community and patient homes. Scheduled hours are Monday through Friday from 8:00 am to 5:00 pm with some flexibility and occasional weekend hours.

In this role, the CHW will work under the direction of designated healthcare professionals to support Aetna members by bridging the gap between community needs and healthcare services. The position demands strong knowledge of health care delivery systems and community resources, excellent interpersonal skills to engage diverse populations, and the ability to navigate complex social and medical situations. The individual will communicate effectively with healthcare providers and members, ensuring cultural, ethnic, and religious preferences are respected in medical care. Organizational and time management skills are essential to coordinate multiple priorities and exercise sound judgment in ever-changing environments.

The role further involves advocating for members to influence healthcare interventions positively and facilitating better mutual understanding among healthcare providers and community members. CVS Health offers this position with a competitive hourly rate ranging from $18.50 to $38.82, depending on experience, education, and location. Job Qualifications

  • high school diploma or GED
  • 1-3 years relevant experience such as community outreach or home visits
  • experience serving Medicaid populations in urban or rural environments
  • strong interpersonal and communication skills
  • ability to work collaboratively on a team
  • proficiency using computers including Excel and electronic health records
  • preferred: Community Health Worker Certification or equivalent training
  • training in Mental Health First Aid, Trauma Informed Care, HIPAA, C.L.A.S. Standards, Motivational Interviewing, and medical terminology
  • meet local state requirements such as first aid, CPR, and TB screen

Job Duties

  • serve as a bridge between members and health care providers by conducting outreach and education
