Job Description

Trinity Health System is a leading healthcare organization committed to delivering high-quality, compassionate, and person-centered care to communities across metro-Detroit and beyond. With a strong presence in the region, Trinity Health System operates multiple clinics, hospitals, and specialized healthcare facilities, including the Cabrini Clinic located at 1234 Porter Street, Detroit, MI. The system prides itself on its mission to honor the dignity of every individual, fostering an inclusive environment that embraces diverse backgrounds, talents, and experiences. As part of its comprehensive network, Trinity Health System provides various services that meet the diverse healthcare needs of the communities it serves, emphasizing cultural competence, accessibility, and patient advocacy. The role of the Community Health Worker (CHW) at Cabrini Clinic is integral in bridging the gap between healthcare providers and the community. This full-time, day-shift position primarily focuses on outreach and connecting patients within Trinity Health's Detroit clinics and partner organizations to essential social and health services. The CHW serves as a crucial liaison who facilitates access to healthcare by providing culturally competent assistance, education, and advocacy to vulnerable populations. This role is well-suited for individuals passionate about community engagement and dedicated to improving health outcomes through personalized support and education. The Community Health Worker conducts outreach activities aimed at enhancing health knowledge and self-sufficiency among community members. By engaging in one-on-one interactions, screenings, telephone contacts, and referrals coordination, the CHW helps individuals navigate complex healthcare and social services systems. A strong emphasis is placed on trauma-informed care, ensuring that patients and families receive sensitive and effective support tailored to their unique situations. Additionally, the CHW promotes the enrollment of eligible individuals into federal, state, and local assistance programs, thereby increasing healthcare accessibility. Working collaboratively with community agencies, schools, and internal healthcare teams, the Community Health Worker assists in delivering health education and wellness promotion while respecting cultural beliefs and practices. This position requires strong interpersonal skills to effectively communicate and advocate for patients within a multidisciplinary environment. Furthermore, maintaining up-to-date knowledge of relevant healthcare laws, program eligibility, and community resources is vital for the role. Trinity Health System values its employees by offering competitive compensation and a comprehensive benefits package that includes medical, dental, vision coverage, paid time off, life insurance, disability, and retirement savings plans with employer contributions. The CHW position also presents ample opportunities for professional growth and advancement within the expansive Trinity Health network. The organization is deeply committed to fostering an inclusive workplace and provides equal employment opportunities regardless of race, color, religion, sex, sexual orientation, gender identity, national origin, disability, veteran status, or other protected statuses. This role is especially beneficial for Spanish-speaking candidates who can effectively communicate within Detroit's diverse communities, enabling the CHW to better serve the population’s needs and improve health equity throughout the region.
