Specializing in home-based healthcare services, Porter employs Nurse Practitioners who visit patients in their homes across Michigan to provide thorough, compassionate, and individualized care that surpasses what a standard fifteen-minute office visit can achieve. Porter is currently seeking a Clinical Provider Engagement Partner to become a vital member of its Clinical Operations Team. This hybrid, field-based role demands an individual based in Michigan, ready to travel regularly across different physician offices, primary care practices, and clinical teams throughout the state. The Clinical Provider Engagement Partner serves as the primary liaison between Porter and the provider community, representing Porter’s mission to improve patient care through trust-building and collaboration with healthcare providers. The ideal candidate will have hands-on clinical experience and a deep understanding of provider challenges within busy practices, ensuring they can empathize and effectively communicate the value Porter’s services bring to both providers and their patients.

In this role, you will foster trusted relationships with primary care providers, care coordinators, office managers, and clinical leaders, becoming an indispensable partner within the practices you serve. You will educate providers on how Porter’s in-home Nurse Practitioner assessments lead to improved care quality by identifying undiagnosed conditions, closing critical care gaps, and enhancing patient engagement. Your role also includes coordinating shared care efforts to streamline seamless communication and collaboration between Porter’s clinical team and provider offices. Your consistent presence and proactive problem-solving will ensure providers feel supported and confident that Porter adds tangible value to their practice and patient outcomes.

You will be the vital link sharing provider feedback internally to inform Porter’s program development and strategy, ensuring the services continually evolve to meet clinical needs. The position also involves tracking engagement trends and practice-level outcomes to optimize Porter’s impact and identify opportunities for program growth. This role requires strong self-organization, interpersonal skills, and the ability to manage a geographic territory independently while maintaining personal relationships. With a valid driver’s license and reliable transportation, you will navigate local travel across Michigan to fulfill this mission.

Job Requirements

  • Active, unencumbered Michigan RN license or current case management certification
  • Minimum of three years clinical or case management experience
  • Proven ability to build trusting relationships with healthcare providers
  • Strong communication and interpersonal skills
  • Commitment to patient-centered care
  • Ability to work independently with self-organization
  • Willingness to travel regularly throughout Michigan
  • Valid driver’s license and reliable transportation

Job Qualifications

  • Active, unencumbered Michigan RN license or current case management certification
  • Three or more years of clinical or case management experience
  • Experience in field-based outreach or care coordination is a strong plus
  • Strong interpersonal and communication skills
  • Commitment to improving outcomes for complex, underserved patients
  • Ability to self-organize and manage geographic territory independently
  • Valid driver’s license and reliable transportation

Job Duties

  • Build trusted, lasting relationships with PCPs, care coordinators, office managers, and clinical leadership
  • Help providers understand how Porter’s in-home NP assessments support their patients
  • Show providers the tangible impact Porter has on shared patients including better documented complexity and more comprehensive visits
  • Collaborate with provider offices to identify shared members who could benefit from Porter’s services
  • Support seamless coordination between Porter’s clinical team and provider practices
  • Serve as a consistent, reliable point of contact answering questions and solving problems
  • Represent provider perspectives internally to inform program development
