The role of Clinical Nurse II at Duke Signature Care is an integral position within this team, offering an exciting opportunity for experienced nursing professionals to actively engage in providing high-quality ambulatory care. This position involves independent planning and delivery of professional nursing care, consistent with established medical and nursing plans of care, policies, and procedures. The Clinical Nurse II functions within the scope defined by the North Carolina Board of Nursing and provides services not only to patients but also supports their families with educational and emotional assistance.
Key responsibilities for this role include conducting patient assessments, managing telephone triage, coordinating care needs, and assisting providers during clinical procedures such as pap smears and wound care. This role demands clinical proficiency in diverse nursing interventions, from administering medications and performing EKGs to phlebotomy and patient education.
The Clinical Nurse II participates actively in daily team huddles to coordinate care plans efficiently and contributes to ongoing professional development by maintaining necessary certifications and competencies. The role schedules patient visits, manages supplies and equipment, and ensures a safe clinical environment. This care provision spans acute and chronic care needs within an ambulatory, concierge-style primary care medical home setting.
The position requires a minimum of an Associate Degree in Nursing or Nursing Diploma, with encouragement toward achieving a Bachelor of Science in Nursing (BSN) within a defined timeframe for academic progression. A minimum of two years of clinical nursing experience is required, with preference given to nurses with backgrounds in adult medical-surgical, emergency, critical care, cardiac, or ambulatory care settings. Nurses must hold current or multi-state compact RN licensure for North Carolina and possess Basic Life Support (BLS) certification.
This full-time role operates on a Monday through Friday day shift schedule, with no weekend or holiday shifts, providing a balanced work-life arrangement. Job Requirements
- Graduation from an accredited Associate Degree in Nursing or Nursing Diploma program
- Registered nurse licensure in the state of North Carolina or compact RN licensure
- Basic Life Support (BLS) certification
- Minimum of 2 years clinical nursing experience

- Willingness to enroll in BSN program within two years of start date if not already obtained

Job Duties
- Participate in daily huddles with providers and clinical team to coordinate patient care needs
- Plan, provide and document professional nursing care in ambulatory settings, utilizing the nursing process, in accordance with physician orders and established policies and procedures
- Perform patient assessment on intake and documentation utilizing double identifiers including reason for visit, vital signs, medication reconciliation, falls risk, goals, learning assessment, PHQ2/9, SDOH, patient screening for immunizations, and appropriate forms
- Assist/chaperone provider with procedures (pap smear, breast/rectal exams, shave biopsy, intra-articular injection/aspiration)
- Collect and analyze patient data, make recommendations for nursing care plans, evaluate nursing care provided, document and communicate appropriately nursing actions taken and patient responses
- Perform EKGs, ear lavage, administer medications (PO, IM, ID, SQ, topical, inhalation), immunizations, nebulizer treatments, suture removal, wound dressings and pulse oximetry as needed
- Monitor and initiate corrective action to maintain the environment of care, including equipment and material resources
- Delegate task and supervise activities of unlicensed assistive personnel
- assist other nursing personnel in the delivery of patient care
- Coordinate patient movement throughout clinic to ensure consultations, procedures, and other forms of care are achieved in a timely manner in accordance with physician orders
