We are currently seeking a Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) to join our Chronic Care Management team in a full-time remote position based in California, starting August 17, 2026. This role is vital in delivering high-quality patient care through telephonic outreach and clinical coordination. As an LVN/LPN at Nsight Health, you will operate in a fast-paced clinical call center environment, managing both inbound and outbound patient interactions. You will conduct comprehensive assessment calls, assist with developing and educating patients about their individualized care plans, perform Behavioral Health Integration assessments, and help troubleshoot medical device issues. Your role directly influences patient engagement and health outcomes by ensuring timely and effective communication between patients and healthcare providers.
A key aspect of this position involves the active and proficient use of artificial intelligence tools such as Claude, ChatGPT, and workflow automation platforms to enhance clinical documentation and care coordination. The ideal candidate will hold an active California LVN license in addition to a multistate Compact LVN license, ensuring compliance with state and national nursing standards. You will maintain an expected volume of 70 to 90 outbound calls each day, efficiently managing clinical inquiries and routing non-clinical calls to the appropriate departments. Precision and accuracy in documenting clinical encounters in real-time within our electronic medical records (EMR) platform are essential to this role.
This position requires adaptability to fluctuating call volumes, evolving clinical processes, and varying workflows, including CCM adherence calls, patient onboarding, and chart reviews. Job Requirements
- Minimum fiber internet speed of 50 Mbps download and 10 Mbps upload

Job Qualifications
- Active LPN/LVN Compact license
- Active unrestricted LVN license issued by California BVNPT and multistate Compact LPN/LVN license from primary state of residence
- Passionate about patient care with empathy and compassion
- Strong communication and organizational skills
- Proficient with computers EMRs and telehealth tools
- At least one year of nursing experience preferred especially in RPM telehealth or chronic care
- Previous call center teleservice or high-volume phone-based work experience preferred
Job Duties
- Operate in a high-volume clinical call center environment managing inbound and outbound telephonic patient interactions
- Conduct outbound phone calls to check in on patients and address health concerns maintaining 70 to 90 calls per day
- Handle inbound phone calls efficiently triaging and routing based on clinical urgency
- Route non-clinical inbound calls to appropriate departments ensuring minimal hold times
- Conduct and document monthly Chronic Care Management care plan calls providing comprehensive care coordination
- Collaborate with providers to coordinate timely and effective patient care via telephonic communication
- Accurately document all patient interactions in real time ensuring documentation completion during each call
- Consistently meet or exceed performance metrics related to care quality patient engagement call handle times and adherence to protocol
- Adapt to fluctuating call volumes shifting priorities and evolving clinical processes
- Pivot to different workflows including CCM adherence calls patient onboarding and chart reviews particularly at the end of each month
- Maintain compliance with company policies and applicable regulations
- Perform other duties as assigned
