The role of Hospital Liaison is a field-based position located primarily in the Eastern Region of North Carolina, with an emphasis on Raleigh (Wake County). This position requires a candidate who can effectively coordinate between hospital emergency departments and internal teams involved in care management and placement, ensuring timely and smooth discharge planning for youth awaiting placement. The role allows for a flexible work environment combining fieldwork and occasional office attendance, fostering responsiveness and direct engagement with client sites. Hospital Liaisons serve as the pivotal point of contact, managing communications related to youth who are boarding in the hospital without an identified discharge plan or placement.

Key responsibilities include coordinating communication with hospital staff such as social workers, case managers, nursing leadership, and security to share status updates and address safety concerns. This role involves organizing and facilitating internal and external staffing meetings, preparing agendas, documenting decisions, and tracking follow-up actions. Hospital Liaisons provide routine updates on placement status, risks, and discharge barriers to internal leadership and involved teams, ensuring the information is current and accurately accessible. The position demands strong coordination skills, a calm and solutions-oriented approach, and the ability to handle multiple active cases efficiently. The liaison supports escalation processes when complex safety issues or systemic barriers arise, ensuring intervention by higher leadership as necessary.

Candidates must have a BA/BS degree with at least one year of experience working with the target population or related community organizations, or an equivalent combination of education and experience. Preferred skills include experience with youth in crisis, emergency department boarding, psychiatric placement, or child welfare and juvenile services, along with familiarity with community-based placements and levels of youth care. This role requires strong documentation, communication, and collaboration abilities to navigate complex healthcare and social service systems.

Work conditions blend remote and office settings with frequent phone, email, and virtual coordination. Periodic onsite hospital visits may be required based on program needs. Candidates should reside within reasonable commuting distance to the designated field locations due to the hybrid nature of the position. Job Requirements

Job Duties

  • Serve as the primary point of contact for hospitals/EDs regarding youth who are boarding and awaiting placement
  • Coordinate and maintain communication with ED staff regarding status updates, safety concerns, and barriers to discharge
  • Coordinate internally with care management and placement teams to align on next steps, documentation, referrals, and placement efforts
  • Schedule, organize, and facilitate staffing meetings including preparing agendas, distributing meeting links, documenting decisions, and assigning follow-up tasks
  • Provide clear, routine written updates to internal leadership and involved teams on each youth's placement status, risks/safety concerns, and outstanding needs
  • Track each youth's boarding timeline, current location/status, and key placement/discharge barriers, ensuring information is current and accessible
  • Support escalation processes when safety concerns, service gaps, or systemic barriers require leadership involvement
  • Ensure communication is accurate, timely, trauma-informed, and aligned with confidentiality and information-sharing requirements
  • Collaborate with hospitals and internal partners to promote continuity of care and reduce avoidable delays in discharge/placement
