With a strong foundation in the healthcare sector, Advanced Care Partners emphasizes employee development and patient-centered care, ensuring that their caregivers feel valued and empowered to make a difference in the lives of those they serve.
The role of a Certified Nurse Assistant/Home Health Aide (CNA/HHA) with Advanced Care Partners involves delivering essential daily care to medically fragile individuals in a variety of home and community settings. This position is ideal for compassionate and patient-focused individuals who want to make a meaningful impact on the lives of children and adults needing specialized support. The schedule options available include full-time, part-time, or PRN (as needed), providing flexibility to those balancing other commitments. The company offers competitive compensation starting at $16 per hour, with additional hourly differentials available during weekends, nights, and holidays to reward dedicated caregivers who take shifts during these critical times. Employees benefit from weekly pay, making financial planning straightforward and predictable.
Caregivers in this role are expected to assist patients with daily living activities such as grooming, bathing, and toileting, which are crucial for the patient’s comfort and hygiene. Additional responsibilities include assisting with medication, taking and recording vital signs, and assessing home environments to ensure they meet patients' safety and health needs. Emotional support is a vital aspect of the role, as caregivers provide encouragement and companionship to patients and their families, fostering a caring and trusting relationship. The role may also involve administering First Aid and CPR when necessary, and educating patients and families about home health practices to promote well-being and independent living.
Advanced Care Partners supports its caregivers with 24/7 on-call assistance and promotes a team-oriented approach to home health care. Working directly in patients’ homes or schools minimizes commute times and allows caregivers to build strong relationships with those they serve. The company’s comprehensive benefits package includes medical, dental, vision, life, and disability insurance, along with paid time off, which collectively help maintain caregivers’ health, financial stability, and work-life balance. Advanced Care Partners is committed to creating an environment where CNA/HHA professionals feel supported, valued, and rewarded for their contributions within a growing and reputable healthcare organization.
