Fox Trail Memory Care is a specialized memory care community dedicated to providing compassionate, personalized care to individuals living with memory impairments such as Alzheimer's and dementia. As a boutique-style home, Fox Trail offers a small, intimate setting with a low resident-to-caregiver ratio, enabling caregivers to build meaningful and lasting relationships with residents. The Certified Home Health Aide (CHHA) role at Fox Trail Memory Care is a full-time position designed for compassionate individuals who are passionate about delivering hands-on care to those with memory care needs. Operating on a schedule of 32 hours per week—Monday, Thursday, Friday, and every other weekend from 7:00 AM to 3:00 PM—this role allows caregivers to maintain a balanced work-life routine while making a real difference in residents’ lives. CHHAs at Fox Trail support residents with a variety of daily living activities including bathing, dressing, grooming, toileting, and mobility assistance. They also help with meal preparation, feeding, hydration, and light housekeeping, maintaining a safe and comfortable home environment.
In addition to physical care, the CHHA role emphasizes emotional support, companionship, and meaningful engagement. Caregivers are encouraged to build genuine, supportive relationships with residents to enhance their well-being and quality of life. They lead or assist with social activities to promote interaction and community involvement among residents. Monitoring residents' basic health conditions and promptly reporting any changes to the Community Director ensures that the care team can address health needs effectively.

Job Duties
- Provide hands-on assistance with activities of daily living including bathing, dressing, grooming, toileting, and mobility
- Assist with meal preparation, feeding, and hydration
- Monitor basic health status and report changes to the Community Director
- Offer companionship, emotional support, and reassurance to residents
- Lead or assist with engaging activities and social interactions
- Perform light housekeeping and laundry to maintain a safe, comfortable home
- Safely assist with resident transfers and mobility as needed
- Support a safe, nurturing environment that meets residents' physical and emotional needs
