Job Requirements

  • Current RN licensure in the state of Florida
  • Minimum of three years acute care hospital nursing experience
  • Preferred certification in case management or utilization review
  • Experience with InterQual software preferred
  • Ability to communicate effectively with patients, families, and healthcare professionals
  • Demonstrated organizational skills and ability to manage multiple priorities
  • Commitment to ethical practice and patient advocacy

Job Qualifications

  • Registered nurse (RN) with current state licensure
  • Bachelor of Science in Nursing (BSN) preferred
  • Minimum of three years clinical acute care hospital nursing experience including Med-Surg, Telemetry, PACU, PCU, ICU, CVICU or similar units
  • Certification in case management or utilization review preferred
  • Experience with InterQual utilization review preferred
  • Strong communication and coordination skills
  • Ability to work effectively in multidisciplinary teams
  • Commitment to patient-centered care
  • Familiarity with rehabilitation nursing standards and practices

Job Duties

  • Provide program orientation to patients, families, and caregivers including the case manager's role, rehabilitation philosophy, continued stay and discharge criteria, Medicare and insurance benefits, grievance procedures, treatment plan process, and rights and responsibilities
  • Act as the coordinator of patient, family, and caregiver education promoting their participation in team discussions related to plans, goals, and status through family conferences and interactions
  • Ensure the implementation of patient treatment plans that support their strengths, abilities, needs, and preferences and facilitate patient involvement throughout the rehabilitation process
  • Document the findings of Discharge Planning Evaluation and psychosocial assessments and communicate social, financial, or discharge needs and preferences of the patient, family, or caregiver
  • Promote consistent, positive patient interactions that advance unparalleled patient service
  • Collaborate with rehabilitation team members including the Rehab Program Director and Facility Case Management Director to ensure coordinated care
  • Participate in continuous quality improvement initiatives for patient care and rehabilitation services
