The role of the RN Case Manager at GWUH is a critical position responsible for the comprehensive management of patient care throughout their hospital journey, from admission to post-discharge. This role demands a highly skilled professional capable of utilizing clinical expertise, critical thinking, and case management principles to evaluate, plan, implement, coordinate, and monitor individualized patient care plans. The Case Manager works diligently to facilitate smooth, efficient, and effective transitions of care across various settings, ensuring patient needs are met while promoting quality outcomes and cost-effectiveness. Interdisciplinary collaboration is key to this role, requiring seamless communication and cooperation with medical professionals, social workers, and external agencies.

Job Requirements

  • Registered Nurse, BSN required
  • Current licensure in the District of Columbia required
  • 2-3 years of recent acute care experience
  • Ability to communicate effectively both orally and in writing
  • Strong organizational and problem-solving skills
  • Ability to maintain confidentiality
  • Ability to adapt to multiple and changing priorities

Job Qualifications

  • Registered Nurse, BSN required
  • Current licensure in the District of Columbia required
  • 2-3 years of recent acute care experience
  • Case Management Certification preferred
  • Effective oral and written communication and interpersonal skills
  • Strong organizational and problem-solving skills
  • Ability to maintain confidentiality
  • Ability to adapt to multiple and changing priorities

Job Duties

  • Assess, plan, implement, coordinate, monitor and evaluate patient care plans from admission to post-discharge
  • Coordinate and negotiate care for complex patients to achieve quality and cost-effective outcomes
  • Collaborate with interdisciplinary staff internal and external to the organization
  • Participate in quality improvement and evaluation processes related to case management
  • Facilitate seamless and effective transitions of care across the continuum
  • Utilize clinical knowledge and critical thinking to resolve patient care challenges
  • Ensure compliance with regulatory and organizational standards in case management
