Job Requirements
- RN with current state licensure
- three years clinical, hospital nursing experience required
- ability to perform case management or critical care nursing
- excellent communication and coordination skills
- ability to work effectively within a multidisciplinary team
- commitment to patient-centered care
- physical capability to meet job demands
Job Qualifications
- RN with current state licensure
- BSN preferred
- three years clinical, hospital nursing experience required with preference given to those with 2 years of case management experience or 2 years of critical care experience
- certification in case management or utilization review preferred
- InterQual experience preferred
Job Duties
- Manage rehabilitative care to ensure optimum patient outcomes
- provide, coordinate, and direct care specific to the needs of each rehab patient
- collaborate with the Rehab Program Director and Facility Case Management Director regarding departmental functions
- coordinate efforts within the rehabilitation team
- participate in patient assessments and care planning
- ensure compliance with hospital policies and healthcare regulations
- communicate effectively with patients, families, and multidisciplinary teams
