Job Requirements

  • RN with current state licensure
  • three years clinical, hospital nursing experience required
  • ability to perform case management or critical care nursing
  • excellent communication and coordination skills
  • ability to work effectively within a multidisciplinary team
  • commitment to patient-centered care
  • physical capability to meet job demands

Job Qualifications

  • RN with current state licensure
  • BSN preferred
  • three years clinical, hospital nursing experience required with preference given to those with 2 years of case management experience or 2 years of critical care experience
  • certification in case management or utilization review preferred
  • InterQual experience preferred

Job Duties

  • Manage rehabilitative care to ensure optimum patient outcomes
  • provide, coordinate, and direct care specific to the needs of each rehab patient
  • collaborate with the Rehab Program Director and Facility Case Management Director regarding departmental functions
  • coordinate efforts within the rehabilitation team
  • participate in patient assessments and care planning
  • ensure compliance with hospital policies and healthcare regulations
  • communicate effectively with patients, families, and multidisciplinary teams
