The Weekend Case Manager position at Houston Methodist is designed to oversee and ensure safe, timely, and efficient patient discharges and transitional care during weekend hours when hospital operations continue but support services may be limited. This role is crucial in maintaining seamless care coordination and facilitating the transition of patients from hospital to home or other care settings during these critical times. The Weekend Case Manager works full time in 10-hour shifts every Thursday through Sunday, from 8 AM to 6 PM, providing continuity of care and maintaining the quality and safety of patient discharges.

This position is held by a Registered Nurse (RN) who plays an essential role in assessing the clinical condition of patients and planning comprehensive case management, including discharge planning and care transitions for targeted patient populations on specific hospital units or service lines. The Weekend Case Manager collaborates closely with physicians and a multidisciplinary healthcare team to coordinate clinical care effectively while upholding compassionate and patient-centered care. The responsibilities include jointly managing discharge planning and coordination of care with social workers, ensuring all admissions and continued stays meet medical necessity, and communicating clinical information to third-party payors and post-acute care providers. The ultimate goal of the Weekend Case Manager is to guarantee that patients experience a safe and smooth transition through the healthcare system, thus achieving desired treatment outcomes.

The role demands a highly skilled individual with a deep understanding of Medicare, Medicaid, managed care requirements, discharge planning, utilization management, case management, and post-acute community resources. The Weekend Case Manager must be proficient in clinical problem-solving, critical thinking, and documentation, alongside being well-versed in technology including Microsoft Office applications. Effective communication skills are imperative for liaising with patients, families, physicians, and healthcare providers to address any clinical or ethical issues and to ensure discharge processes are timely and efficient.
