The Case Manager, Registered Nurse (Field-Based) role based in Mercer County, New Jersey, offers a dynamic and impactful opportunity within CVS Health's clinical team. This full-time remote position involves up to 75% travel within the field, operating Monday through Friday from 8:00 AM to 5:00 PM EST. The primary responsibility of this role is to support members in Mercer County and the surrounding areas by delivering personalized, high-quality, and cost-effective care. The Case Manager leverages clinical expertise to assess member needs comprehensively, coordinate physical, behavioral, and psychosocial services, and develop individualized care plans that support the wellbeing and independence of members.

This role requires conducting in-home, face-to-face assessments for members enrolled in programs such as Managed Long-Term Services and Supports (MLTSS) and Dual Special Needs Programs (D-SNP/FIDE). The Case Manager evaluates member needs and coordinates a range of services, including adult or pediatric medical daycare, personal care assistants, nursing facility placements, and MLTSS program enrollment. A vital part of the role involves partnering with members, caregivers, primary care providers, and interdisciplinary teams to implement care plans that facilitate safe transitions of care. The position demands the accurate and prompt documentation of all case activities in electronic health records and the application of strong clinical judgment and critical thinking skills to manage complex care scenarios. Mentoring new team members is also part of the responsibilities once proficiency is attained.

Offered pay for this role ranges from $66,575 to $142,576 annually, reflecting factors like experience, education, and geographical location.
