Job Requirements

  • Candidate must reside within Cook County, Illinois or be willing and able to travel there
  • candidate must possess reliable transportation
  • willingness and ability to travel up to 50 percent of the time within Cook County
  • active and unrestricted Registered Nurse license in Illinois
  • 3+ years clinical nursing experience
  • 2+ years experience using computers and navigating multiple systems
  • ability to work independently
  • effective communication and interpersonal skills

Job Qualifications

  • Associate's Degree in Nursing
  • Registered Nurse license active and unrestricted in Illinois
  • 3+ years clinical practical experience as a Registered Nurse
  • 2+ years experience using personal computers and MS Office Suite applications
  • excellent analytical and problem-solving skills
  • strong communication, organizational, and interpersonal abilities
  • ability to work independently
  • confidence working from home and collaborating virtually
  • Certified Case Manager preferred
  • 2+ years case management, discharge planning, or home health care coordination experience preferred
  • Bachelor’s Degree in Nursing preferred

Job Duties

  • Act as a liaison with members, clients, families, employers, providers, insurance companies, and healthcare personnel
  • implement and coordinate case management activities for catastrophic and chronic cases across the continuum of care
  • interact with members telephonically or in person, including home and office visits
  • assess and analyze medical and vocational status of members to develop care plans
  • communicate with members and stakeholders regarding care plans and progress
  • prepare all required documentation of casework activities
  • consult with internal multidisciplinary teams to maximize health outcomes
  • outreach to treating physicians or specialists as appropriate
  • provide educational and prevention information
  • apply laws and regulations related to rehabilitation services
  • conduct evaluations of member needs and benefit eligibility
  • utilize case management processes compliant with policies
  • facilitate condition management and optimize wellness and return to work
  • develop proactive action plans to enhance short- and long-term outcomes
  • monitor member progress toward desired outcomes through assessments and evaluations
