The Aetna team, a division within CVS Health, is focused on serving dual eligible populations—individuals enrolled in both Medicare and Medicaid—who face complex health and social challenges. The Case Manager position within the Aetna team is pivotal in elevating patient care by leveraging a collaborative process that includes assessment, planning, facilitation, care coordination, evaluation, and advocacy. This role demands a compassionate approach combined with excellent communication skills to effectively address the full continuum of members' health care needs. Responsibilities include liaising with clients, families, providers, and insurance companies, coordinating case management activities for catastrophic and chronically ill members, and ensuring a smooth continuum of care across various settings.

Case Managers will engage directly with members through various methods, including telephonic and in-person meetings, potentially visiting homes, worksites, or physician offices to provide ongoing support. By analyzing the medical and vocational statuses of clients, case managers develop individualized plans aimed at optimizing wellness, medical outcomes, and timely return to work or optimal functioning. The role also involves working with interdisciplinary teams, conducting outreach to treating physicians, providing educational resources, and ensuring compliance with all applicable laws and insurance guidelines.

This full-time role requires a dedicated professional residing in or willing to travel frequently to Cook County, Illinois. The typical weekly hours amount to 40 with a competitive pay range from $66,575.00 to $142,576.00 annually. Additionally, candidates may be eligible for bonuses or incentive programs. Job Requirements

  • Candidate must reside within Cook County, Illinois or be willing and able to travel there
  • Must possess reliable transportation and be willing to travel up to 50% of the time within Cook County
  • Active and unrestricted Registered Nurse license in Illinois
  • At least 3 years clinical experience as a Registered Nurse
  • Minimum 2 years’ experience using personal computers and MS Office applications
  • Confidence working independently from home
  • Strong analytical and problem-solving abilities
  • Effective communication and organizational skills
  • Ability to work independently without supervision

Job Qualifications

  • Associate’s Degree in Nursing required
  • Bachelor’s Degree in Nursing preferred
  • Active and unrestricted Registered Nurse license in Illinois
  • Minimum 3 years clinical practical experience as a Registered Nurse
  • Experience using personal computers and MS Office suite applications
  • Excellent analytical and problem-solving skills
  • Effective communication, organizational and interpersonal skills
  • Ability to work independently
  • Certified Case Manager preferred
  • 2+ years experience in case management, discharge planning or home health care coordination preferred

Job Duties

  • Act as a liaison with members, clients, families, employers, providers, insurance companies, and healthcare personnel as appropriate
  • Implement and coordinate all case management activities relating to catastrophic cases and chronically ill members across the continuum of care
  • Interact with members telephonically or in person, including home, worksite, or physician office visits
  • Assess and analyze medical and vocational status of injured or chronically ill members and develop plans of care to optimize wellness and medical outcomes
  • Communicate effectively with members and stakeholders
  • Prepare documentation of case work activities
  • Collaborate with internal multidisciplinary teams to maximize health outcomes
  • Conduct outreach to treating physicians or specialists as needed
  • Provide educational and prevention information
  • Ensure compliance with laws, regulations, and insurance carrier requirements
  • Evaluate members’ needs and benefit eligibility
  • Monitor progress towards desired outcomes and adjust plans accordingly
