As a Case Manager, you will utilize a strategic, multidimensional process of assessment, planning, facilitation, care coordination, evaluation, and advocacy, aimed at optimizing quality and cost-effective outcomes for members.

Operating primarily within Cook County, Illinois, this full-time role requires flexibility and a willingness to travel up to 50% of the time to meet members in multiple settings, such as homes, workplaces, or physician offices. This mobility is essential to effectively coordinate care and foster stronger connections between all parties involved in member treatment plans.

The Case Manager will prepare all necessary documentation, consult with multidisciplinary teams, communicate with physicians regarding care plans, and provide education focused on prevention and optimal medical outcomes. Compliance with relevant laws, regulations, and carrier-specific instructions is mandatory, along with ongoing evaluation of members’ needs, eligibility, and responses to interventions.

This role offers a competitive salary range from $66,575 to $142,576 annually and includes eligibility for CVS Health bonus, commission, or short-term incentive programs.
