The Case Manager also performs thorough assessments of medical and vocational status, develops individualized care plans, and facilitates the integration of clinical tools and community resources to optimize health outcomes and timely return to work or function. The role demands a self-driven and independent professional who is comfortable working from home and connecting with teams virtually through various technology platforms.
Located primarily in Cook County, Illinois, this position offers a competitive salary range from $66,575 to $142,576 per year, complemented by eligibility for bonuses, commissions, or short-term incentive programs. Job Requirements
- Candidate must reside within or be willing to travel to Cook County, Illinois
- Must possess reliable transportation and be willing to travel up to 50% within Cook County
- Active and unrestricted Registered Nurse (RN) license in Illinois
- Minimum 3 years clinical practical experience as a Registered Nurse
- Minimum 2 years experience navigating multiple computer systems and MS Office Suite
- Confidence working independently from home
- Excellent analytical and problem-solving skills
- Strong communication, organizational and interpersonal skills
- Ability to work independently
Job Qualifications
- Associate's degree in nursing required
- Bachelor's degree in nursing preferred
- Active and unrestricted Registered Nurse (RN) license in Illinois required
- Minimum of 3 years clinical practical experience as a Registered Nurse
- Minimum of 2 years experience using personal computers and MS Office applications
- Excellent analytical and problem-solving skills
- Effective communication, organizational, and interpersonal skills
- Ability to work independently
- Certified Case Manager preferred
- Minimum of 2 years case management, discharge planning or home healthcare coordination experience preferred
Job Duties
- Act as a liaison with members, clients, families, employers, providers, insurance companies, and healthcare personnel as appropriate
- Implement and coordinate all case management activities relating to catastrophic cases and chronically ill members across the continuum of care
- Interact with members telephonically or in person, including home and physician office visits
- Assess and analyze medical and vocational status of members
- develop and facilitate individualized care plans
- Communicate with members and stakeholders to ensure coordinated care
- Prepare all required documentation for case management activities
- Collaborate with internal multidisciplinary teams to optimize health outcomes
