This full-time Case Manager position within the Aetna team offers a unique opportunity to make a life-changing impact on members facing significant healthcare and social challenges. The Case Manager also performs thorough assessments of medical and vocational status, develops individualized care plans, and facilitates the integration of clinical tools and community resources to optimize health outcomes and timely return to work or function. Located primarily in Cook County, Illinois, this position offers a competitive salary range from $66,575 to $142,576 per year, complemented by eligibility for bonuses, commissions, or short-term incentive programs. The company reimburses mileage costs related to travel, acknowledging the requirement for up to 50% travel within Cook County to meet member needs.
