The Case Manager role at Optum Insight is vital in coordinating patient services through an interdisciplinary, patient-centered approach that spans the continuum of care. This position involves the assessment of patient needs for appropriate levels of admission, continued hospitalization, and discharge planning. Concurrent case management ensures that patients receive timely and coordinated care tailored to their clinical and psychosocial needs. The role is based onsite at a hospital facility in the Boulder, Colorado area, reflecting the importance of direct patient interaction and collaboration with hospital staff. This is not a remote position. The Case Manager plays a crucial part in discharge planning starting from admission and continually reassesses to facilitate smooth transitions to the next level of care. The incumbent will work closely with physicians, nursing staff, social workers, therapists, contracted payors, and family members to develop and execute effective discharge and care plans that optimize patient outcomes and resource utilization.

The position requires a commitment to compliance with regulatory standards such as CMS and Joint Commission requirements and the ability to navigate federal and state healthcare regulations, including managed care and Medicare plans. Key responsibilities also include monitoring quality and risk management issues, communication and reporting to multidisciplinary teams, and supporting the hospital’s operational needs by floating to various departments when necessary. This professional role demands strong interpersonal skills, leadership by example, and the ability to function independently with excellent organizational skills. The schedule includes working one weekend shift every two weeks as well as other days as needed.

The role is suitable for Registered Nurses with current licenses in Colorado or qualified Social Workers (Licensed Clinical Social Workers or those holding a Master of Social Work). At least two years of acute care clinical experience is desirable, along with proficiency in hospital workflows. Competitive hourly pay ranges from $29.00 to $52.00 for full-time employment depending on factors like education, experience, and certifications. Job Requirements

  • Registered Nurse with current, active, unrestricted Colorado license or Licensed Clinical Social Worker in Colorado or Master of Social Work
  • Minimum of 2 years of clinical experience in acute care
  • Live in Boulder CO regional area
  • Willing and able to work one weekend shift every two weeks and additional days as needed
  • Ability to work onsite at hospital facility
  • Pass required drug test
  • Comply with all regulatory, CMS, and Joint Commission standards

Job Qualifications

  • Registered Nurse (RN) with current, active, unrestricted Colorado license or Licensed Clinical Social Worker (LCSW) or Master of Social Work (MSW)
  • Two or more years of clinical experience in acute care setting
  • Knowledge of hospital workflows
  • Case management experience or certification preferred
  • Understanding of community resources, policies, and procedures preferred
  • Strong clinical judgment and problem-solving skills
  • Excellent organizational skills
  • Collaborative interpersonal skills
  • Effective written and verbal communication
  • Proficient with Microsoft Office
  • Ability to learn clinical software applications
  • Proficient typing skills

Job Duties

  • Coordinate patient services through interdisciplinary processes
  • Assess patient needs for admission, continued hospitalization, and appropriate level of care
  • Develop and execute discharge plans in collaboration with physicians, nursing staff, social workers, and payors
  • Facilitate completion of necessary paperwork and medical orders prior to patient transfer
  • Monitor and report quality and risk management issues
  • Participate and facilitate multi-disciplinary care conferences
  • Communicate effectively with patients, families, and healthcare team members
