The Case Manager II-III Youth Diversion position, based in Amarillo, Texas, within the Family & Youth Diversion program, offers a vital role in supporting at-risk youth and their families. This full-time role comes with an annual salary range of $43,305.60 for Case Manager II and $46,009.60 for Case Manager III. The successful candidate will be integral in providing direct care support, including skills training and coordination for youth facing or at risk of crisis episodes that impair functioning in various environments such as home, school, and community. Responsibilities include working closely with families and involved parties to deliver culturally competent, evidence-based interventions tailored to promote stability, resilience, and overall well-being. The role demands a trauma-informed approach, ensuring ethical communication and respect at all times.

Additionally, the Case Manager will engage in group and individual services, collaborate on treatment planning and care coordination, and maintain strong knowledge of community resources to support clients comprehensively. This position requires about half of the work time spent directly with youth, families, and support networks, delivering services both face-to-face and telephonically in various settings throughout the Texas Panhandle region. Ethical adherence to TPC’s Business Code of Conduct is mandatory, with vigilance to report any breaches in standards. Job Requirements

  • Must have a bachelor's degree from an accredited university with a major in psychology, sociology, social work, counseling, or related human services field for QMHP credentialing
  • Must have minimum one year of experience in behavioral health case management for Case Manager II
  • Must have minimum two years of experience in behavioral health case management for Case Manager III
  • Graduate work in a related field may substitute for up to one year of experience
  • Employees may be required to utilize personal mobile phones for two-factor authentication
  • Must have a valid Texas driver's license and be insurable under agency vehicle policy
  • Must be 18 years of age or older

Job Qualifications

  • Bachelor's degree from an accredited university with a major in psychology, sociology, social work, counseling, or related human services field
  • For Case Manager II, minimum one year of experience in behavioral health case management
  • For Case Manager III, minimum two years of experience in behavioral health case management
  • Graduate work in a related field may substitute for up to one year of experience
  • Knowledge of trauma-informed care
  • Ability to provide crisis intervention and support
  • Effective communication and interpersonal skills
  • Experience working with youth and families preferred

Job Duties

  • Maintain ethical and professional communications using respectful Trauma-Informed Care language
  • Serve as a Skills Trainer Specialist providing direct care support and coordination
  • Provide services in individual and group settings
  • Organize culturally competent care activities and collaborate with all participants involved in youth's care
  • Provide evidence-based practices for families and youth at-risk or experiencing crisis episodes
  • Maintain knowledge of community resources, services, and support for families
  • Provide skills training using evidence-based curriculums, assessment and treatment planning, engage family by providing outreach and education, care coordination, and person-centered approaches
  • Deliver services via phone and face-to-face in home and community-based settings
  • Spend at least half of working time directly with youth, family, and/or family supports
  • Adhere to TPC Business Code of Conduct and report concerns regarding ethical or business standards
