The Case Manager position at Neighborhood House is a full-time, non-exempt role primarily responsible for facilitating the care and support needs of aging and disabled clients to help them remain safely and independently in their homes. This role requires a dedicated professional who can navigate a flexible and dynamic atmosphere while working directly with approximately 75 to 85 clients. The Case Manager conducts in-home functional assessments and reassessments, develops personalized care plans addressing Activities of Daily Living such as cooking, cleaning, and bathing, and ensures these plans align with client goals. The role also involves monitoring eligibility for services, managing care plans with periodic reviews, maintaining meticulous and confidential client records, and advocating on behalf of clients and their families to connect them with appropriate resources and services. The Case Manager reports to the Community Health Supervisor and will be expected to participate in clinical case reviews, training sessions, and consultative meetings to continuously improve care delivery. This position also requires some travel within King County to client homes, proficiency in working with interpreters as needed, and flexibility to occasionally work evenings or weekends.

Job Requirements
- Bachelor's degree or associate's degree in related field
- 3-5 years of social work or human services experience focusing on aging, low-income, diverse populations
- bilingual Arabic and Kurdish language skills with English proficiency

Job Qualifications
- Bachelor's degree in social work, behavioral science, or related field with 3 years of experience in social work or human services with a focus on aging, low-income, and diverse populations
- or associate's degree in related field with 5 years of relevant experience
- bilingual in Arabic and Kurdish with English proficiency sufficient for complex documentation and navigating state databases
- demonstrated ability to thrive in a culturally diverse workplace committed to equity and inclusion
- commitment to advancing racial equity and social justice
- experience working with elderly or disabled adults with unstable health, multiple physical or mental health problems, long-term care needs, or substance abuse issues

Job Duties
- Support program outreach activities to inform the community about services available
- conduct in-home functional assessments and reassessments for about 75-85 clients
- collect personal data and analyze information to establish tailored care plans supporting client goals
- review program eligibility annually and as needed
- provide oversight and reviews of care plans
- document all client activities and maintain confidential records
- provide information, referral, and advocacy linking clients and caregivers to appropriate services

- maintain compliance with DSHS Quality Assurance and HIPAA expectations
