We are currently seeking a Case Manager Assistant to join our team and contribute to a seamless continuum of care for our managed care and Medicare residents. This role is integral to coordinating and facilitating interdisciplinary healthcare plans, ensuring that progress reports are timely and accurately communicated to payors as required. The Case Manager Assistant serves as a critical liaison connecting patients, physicians, care teams, payors, and discharge planners. Their role involves closely monitoring the patient’s progress, assessing cost-effectiveness of treatments, and coordinating the admission and discharge activities to ensure quality transitions to the next level of care.

Responsibilities include negotiating appropriate levels of care within contract terms with payor case managers and utilizing pricing tools for non-contracted arrangements. The Case Manager Assistant supports effective communication among all care team members to guarantee smooth patient transitions and timely sharing of accurate discharge plans. An essential part of this role is the development of referral sources, which include hospitals, physicians, insurance companies, and other healthcare professionals. Active participation in marketing action plans, key account meetings, and the assumption of key account management duties under supervision are also expected.

The candidate will be responsible for ensuring care delivery aligns with safety, timeliness, effectiveness, efficiency, equity, and client-centered principles. Monitoring case assignments, reviewing case progress, and making decisions on case closures form the backbone of operational responsibilities. Supporting patients in making informed healthcare decisions and advocating for their clinical needs is a key aspect of this role. Additionally, the Case Manager Assistant will collaborate closely with rehabilitation teams, MDS coordinators, business offices, admissions, and nursing leadership to foster a multidisciplinary approach to patient care. The position also includes tasks such as documentation management, staffing support, and conducting essential department meetings.

Job Requirements

  • Completion of an accredited medical assistant program
  • at least 1-2 years of clinical experience as a medical assistant
  • prior case management or care coordination experience
  • strong communication and organizational skills
  • ability to work collaboratively with multidisciplinary teams
  • proficiency in documentation and case tracking
  • knowledge of managed care and Medicare processes
  • willingness to assist with staffing and training as needed

Job Qualifications

  • Completion of an accredited medical assistant program (CAAHEP or ABHES recommended)
  • certification as a medical assistant (e.g., AAMA, AMCA, NCCT) preferred
  • bachelor's degree in nursing, healthcare administration, social work, or a related field preferred
  • certified case manager (CCM) credential optional but highly desirable
  • at least 1-2 years of clinical experience as a medical assistant, preferably in long-term care or geriatric settings
  • prior experience in case management, care coordination, discharge planning, or utilization review in a healthcare setting (SNF experience strongly preferred)

Job Duties

  • Negotiate appropriate level of care within contract terms with the payor case manager
  • utilize out of contract pricing worksheet for non-contracted arrangements
  • communicate information to the care team and coordinate the patient’s smooth transition to the next level of care
  • obtain accurate information from physicians, patient, and payor source regarding the expected discharge plan and communicate this information to the interdisciplinary team
  • develop referrals from hospitals' social service and discharge planning departments, physicians, case managers, insurance companies and other referral sources
  • participate in marketing action plan and key account meetings and assume key account management responsibilities as directed by their supervisor
  • coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered
  • handle case assignments, review case progress and determine case closure
  • help clients achieve wellness and autonomy
  • facilitate multiple care aspects such as case coordination and information sharing
  • help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options
  • develop effective working relations and cooperate with the medical team throughout the entire case management process
  • interact with patients to keep track of their progress and ensure satisfaction
  • record case information and complete all necessary forms accurately
  • help with staffing if necessary
  • ensure that daily skilled documentation is in place for skilled patients
  • work collaboratively with rehab, MDS coordinator, business office, admissions, ADONs and DONs
  • conduct PPS/Skilled Kaiser meetings as scheduled
  • prepare and update MCR/Skilled tracking sheet
