Job Requirements
- High school diploma or equivalent
- Required training or licensure as per state and federal laws
- Experience as a caregiver with at least 3 years
- Must be able to pass a background check and drug screen
- Valid driver’s license and reliable vehicle
- Ability to lift and assist with mobility
- Available for work in Mesa, AZ 85275 and willing to relocate before starting work

Job Duties
- Provide direct and indirect care with safety measures
- Maintain effective communication with supervisors
- Follow written care plans and document services provided
- Observe and report changes in client condition
- Adhere to agency policies and emergency procedures
- Submit required documentation in a timely manner
- Adhere to the role as defined in state regulation
