This position is a non-exempt, hourly role available on both a full-time and part-time basis. As a direct support professional within this role, you will be responsible for providing personalized care and assistance within a single program or unit, typically supporting between four to six individuals in residential settings or up to ten individuals in periodic or day programs. The nature of this job demands a compassionate, patient, and reliable individual who is passionate about making a tangible difference in the lives of people with developmental disabilities.

Your key responsibilities will include assisting individuals with daily activities such as personal care, housekeeping, recreational activities, community integration, transportation, shopping, and citizenship. You will actively support skill and behavioral development according to detailed individual service and program plans, including administering and monitoring medications, supporting health maintenance, and ensuring overall well-being. Detailed record-keeping is critical to this role, as you will need to document progress and activities accurately, review logs to stay current on any changes in the individuals' care plans, and maintain strict confidentiality.

Moreover, this role extends beyond routine caregiving. It involves managing individuals’ assets ethically and cost-effectively, fostering healthy relationships with individuals and their families, guardians, and case managers, and adhering to Sevita’s customer service standards. You will be entrusted with reporting any alleged abuse or neglect promptly, following established internal and external protocols, and upholding the rights and dignity of those you serve in line with applicable rights and safety regulations.

Additional responsibilities include accompanying individuals to medical appointments, supporting medication administration and health monitoring, preparing meals aligned with individual dietary needs and preferences, and ensuring the safe and proper use of medical equipment. Employment responsibilities also encompass participating in all required training sessions and team meetings, adhering to scheduling requirements, and promoting teamwork and open communication.

Job Requirements

  • High school diploma or equivalent preferred
  • Six months of experience in human services preferred
  • Must be 18 years of age
  • Valid driver’s license in good standing
  • Car registration and vehicle insurance if providing transportation
  • Completion of all state-required training and certification in mandated timeframes
  • Must maintain valid driver’s license and meet company driving record requirements
  • Ability to pass drug testing prior to and during employment
  • Must agree to and pass criminal background checks and registry screenings
  • Ability to cooperate with inspections, inquiries, and investigations
  • Meet physical requirements to perform heavy work
  • Willingness to work at program locations and accompany individuals into the community

Job Qualifications

  • High school diploma or equivalent preferred
  • Six months of experience in human services preferred
  • Must be 18 years of age
  • Working knowledge of computers
  • Valid driver’s license in good standing
  • Completion of all state-required training and certifications within mandated timeframes
  • Approval by management to provide transportation
  • Ability to maintain confidentiality and uphold ethical standards

Job Duties

  • Provide direct support or assistance in accordance with individual service or program plans
  • Assist with socialization and behavioral development
  • Support personal care, housekeeping, recreational activities, transportation, community orientation, shopping, financial management, citizenship, and other activities of daily living
  • Accurately document progress and activities while maintaining confidentiality
  • Manage individuals’ assets responsibly and within budget
  • Maintain healthy professional relationships with individuals, families, guardians, and case managers
  • Report any alleged abuse or neglect and other incidents according to policy
  • Administer or support self-administration of medications and document accordingly
  • Prepare or assist with meal preparation following planned menus and special diets
  • Participate in training and staff meetings
  • Adhere to employee scheduling policies and maintain punctuality
  • Promote teamwork and open communication
  • Ensure workplace safety and report hazards
  • Drive safely and transport individuals according to local laws
  • Perform daily housekeeping and minor maintenance tasks
  • Participate in safety drills and emergency procedures
