Job Description

The In-Home Caregiver role based in Epping, NH, and Windham, NH, provides non-clinical in-home care services to health plan members and private clients. This position is part-time or per-diem and classified as a 1099 contractor opportunity, which offers caregivers control over their working hours through the CareLinx app. This innovative platform allows caregivers to set their availability, preferences, and goals, then matches them with clients accordingly. Whether seeking steady recurring shifts or flexible per-diem work, caregivers have the autonomy to build their schedules while being supported by a dedicated Care Partner team who assists in finding the best client matches.
This role is ideal for compassionate and professional individuals passionate about making a genuine impact on clients' daily lives. The caregiver assists with activities of daily living including companionship, meal preparation, mobility support, assistance with bathing, grooming, and dressing, medication reminders (non-clinical), and light housekeeping. Caregivers are responsible for accurately documenting care tasks using the CareLinx mobile app, monitoring and reporting any changes in the client’s condition, and communicating regularly with their Care Partner to provide updates and ensure continuity of care. Maintaining client confidentiality and dignity is a core component of this role, as is ensuring a safe and secure environment for the clients.
Additional shift opportunities may include transportation and errand support, hospital discharge escort services, and telehealth navigation to assist clients with virtual doctor visits. This role demands reliability, self-direction, and organizational skills, complemented by technological comfort with smartphone applications.
CareLinx requires caregivers to have a minimum of six months of caregiving experience or relevant education or certification such as CNA, HHA, or MA. A smartphone capable of running the CareLinx app is mandatory, alongside reliable transportation and the ability to communicate effectively in English; bilingual caregivers are highly encouraged to apply. Background checks are standard, and depending on the specific assignment, program-related requirements such as medical clearances, driver’s license checks, and additional documentation may be necessary.

Job Requirements
- Minimum six months of caregiving experience or relevant education/certification such as CNA, HHA, MA, or equivalent
- Access to a smartphone capable of running the CareLinx app
- Ability to communicate in English
- Reliable transportation to and from client homes
- Must pass a background check
- Completion of program-specific training prior to accepting shifts may be required
- Medical clearance such as TB test, physical exam, or drug screening may be required
- Valid driver’s license and MVR check if accepting driving shifts
- Additional documentation may be required based on state law or client contract

Job Duties
- Assist clients with activities of daily living including companionship, meal prep, mobility support, bathing/grooming/dressing, medication reminders (non-clinical), and light housekeeping
- Complete and document all care tasks accurately in the CareLinx mobile app during each shift
- Monitor and report changes in a client’s condition or self-care abilities to the CareLinx clinical team
- Submit timesheets on time through the CareLinx app
- Communicate with your Care Partner regarding any case updates or client changes
- Ensure patient privacy, dignity, and well-being in a safe and secure environment
- Maintain client confidentiality at all times
- Provide transportation and errand support
- Safely accompany members home after hospital discharge
- Assist members with telehealth navigation for virtual doctor visits
