Job Requirements
- Minimum 6 months of caregiving experience or relevant education/certification (CNA, HHA, MA, or equivalent)
- Access to a smartphone capable of running the CareLinx app
- Ability to communicate in English
- Reliable transportation to and from client homes
- Must pass a background check
- Completion of program-specific training prior to accepting shifts if applicable
- Medical clearance such as TB test, physical exam, or drug screening as required
- Valid driver's license and MVR check if accepting driving shifts
- Additional documentation may be required based on state law or client contract
Job Qualifications
- Minimum 6 months of caregiving experience or relevant education/certification such as CNA, HHA, MA, or equivalent
- Ability to communicate in English (bilingual caregivers strongly encouraged to apply)
- Compassionate, patient, and professional attitude
- Committed to supporting members in living independently on their own terms
- Reliable and punctual
- Self-directed and organized with good communication skills
- Comfortable using a smartphone app for work tasks
Job Duties
- Assist clients with activities of daily living (ADLs) including companionship, meal prep, mobility support, bathing/grooming/dressing, medication reminders (non-clinical), and light housekeeping
- Complete and document all care tasks accurately in the CareLinx mobile app during each shift
- Monitor and report changes in a client’s condition or self-care abilities to the CareLinx clinical team
- Submit timesheets on time through the CareLinx app
- Communicate with your Care Partner regarding any case updates or client changes
- Ensure patient privacy, dignity, and well-being in a safe and secure environment
- Maintain client confidentiality at all times
- Provide transportation and errand support as needed
- Escort clients safely after hospital discharge
- Assist clients with telehealth navigation
