The position available is for a Care Manager Licensed Professional (Care Manager - LP) working remotely but requiring residency in or near Cherokee County, North Carolina, or within 40 miles of the NC border to facilitate necessary in-person visits and travel. The Care Manager - LP plays a vital role in providing proactive intervention and coordination of care to eligible Vaya Health members. This position is responsible for conducting comprehensive assessments, collaborating with multidisciplinary care teams, and ensuring appropriate services and support are accessible to members across various health domains such as mental health, substance use, intellectual/developmental disabilities, physical health, pharmacy, long-term services, and other unmet health-related needs.
Care Managers utilize their licensed clinical knowledge to assess member needs, formulate individualized care plans with input from members and care teams, and provide clinical consultation and recommendations for appropriate interventions. The role involves conducting health risk assessments that address biopsychosocial factors, reviewing medication lists to promote continuity of care, facilitating care transitions from hospitals and residential facilities, and diverting institutional placements when possible. Frequent collaboration with Vaya staff, community providers, caregivers, and natural supports is essential to help members achieve their health goals and maintain stability in their communities.
This mobile role requires travel within the service area and working in various locations, including members' home communities. The Care Manager - LP ensures compliance with HIPAA regulations, maintains detailed documentation in Vaya’s administrative health record system, and participates in care team meetings and interdisciplinary collaboration. In addition to clinical duties, the Care Manager supports population health management, prevention programs, and educational initiatives to empower members and families. The position requires advanced problem-solving skills, the ability to navigate complex clinical scenarios, and to engage with diverse populations compassionately and effectively.
Candidates for this role must meet North Carolina residency requirements and hold valid professional licensure, such as Registered Nurse (RN), Licensed Clinical Social Worker (LCSW), Licensed Clinical Mental Health Counselor (LCMHC), or other approved licenses. This ensures the individual can operate within a multi-disciplinary framework and provide clinical assessments, care planning, and advocacy effectively. Job Requirements
- Must live in or near Cherokee County, North Carolina or within 40 miles of the NC border
- valid professional licensure required
- master’s degree in related field
- minimum of two years experience serving behavioral health or long-term care populations (depending on degree)
- ability to travel and conduct in-person visits
- compliance with HIPAA privacy regulations
- strong organizational and problem-solving skills
- ability to use clinical judgment and work independently with multidisciplinary teams
- must meet North Carolina Department of Health and Human Services residency requirements
- physical ability to drive and sit for extended periods
Job Qualifications
- Master’s degree in health, psychology, sociology, social work, nursing or related human services field
- relevant experience working directly with behavioral health or long-term services and supports members
- valid professional licensure such as RN, LCSW, LCMHC, LPA, LCAS, LMFT or equivalent
- ability to perform clinical assessments and care planning
- strong interpersonal and communication skills
- proficiency in Microsoft Office and care management platforms
- knowledge of NC Medicaid program and Medicaid Transformation initiatives preferred
- ability to maintain required training proficiencies
