The role of Care Manager, also known as Designated Care Manager, at Sunrise of Carmichael represents a vital position in fulfilling this mission. This is a hands-on role responsible for delivering the highest standard of quality care and services to residents within assisted living, long-term care, and reminiscence neighborhoods. Care Managers work closely with their designated group of residents to provide physical and emotional support uniquely tailored to each individual's needs as outlined in their Individualized Service Plans (ISPs). The position requires maintaining a safe, comfortable, and home-like atmosphere, ensuring that all care is delivered in strict accordance with federal, state, provincial, and local regulations alongside Sunrise Senior Living’s internal policies, procedures, and standards.

Care Managers play a pivotal role in the holistic wellbeing of residents by continuously observing, documenting, and reporting changes in residents’ conditions, behaviors, and needs. Their responsibilities also extend to collaborating with families and the broader care team to update care plans as necessary. The Care Manager supports residents' independence and dignity while facilitating participation in activities that promote life enrichment and meaningful engagement.

Job Requirements

  • Must be at least 18 years old
  • High school diploma or GED as required
  • CPR and First Aid certification
  • Ability to serve and care for seniors
  • Effective communication and documentation skills
  • Ability to remain calm and handle multiple priorities
  • Compliance with state and federal health regulations
