The role of a Care Manager, sometimes referred to as a Designated Care Manager, at Sunrise Senior Living involves providing exceptional care and support to residents in assisted living, long-term care, and reminiscence neighborhoods. Job Requirements
- Must be at least 18 years of age
- Ability to make decisions in the best interest of residents
- Desire to serve and care for seniors
- Ability to remain calm and manage multiple priorities
- Willingness to comply with infection control and safety procedures
- Ability to complete and pass pre-employment drug test, tuberculosis test, physical evaluation, and background check
- Covid-19 and Influenza vaccination may be required as per applicable laws
- For team members driving Sunrise vehicles, must sign Driver job description and comply with safety duties
Job Qualifications
- High school diploma or GED accepted
- CPR and First Aid certification as required by state or province
- Previous experience working with seniors preferred
- Ability to communicate effectively both verbally and in writing
- Competency in organizational and time management skills
- Demonstrated good judgment, problem-solving, and decision-making abilities
- Commitment to serving seniors with empathy and respect
- Ability to work effectively as part of a team
- Willingness to participate in ongoing training and compliance measures
Job Duties
- Review, read, notate, and initial daily log to document resident information
- Participate in development and monthly updates of Individualized Service Plans (ISPs)
- Responsible for designated group of residents during shifts, conduct physical checks
- Observe, report, and document changes in resident condition
- Respond promptly to security system and resident calls
- Assist with continence management and maintain sanitation
- Engage residents in activities, promote dignity and independence
- Ensure community compliance with safety and occupational health regulations
- Report accidents, unsafe conditions, and hazardous equipment
- Maintain and clean resident rooms and common areas
- Serve and support residents during meals
- Follow resident diets and document dining habit changes
- Participate in reminiscence neighborhood activities including life skills enrichment
- Contribute to team goals and participate in training and meetings
