Job Description

The Care Manager, also referred to as the Designated Care Manager, plays a critical role within the assisted living and long-term care neighborhoods, including reminiscence care settings. This role is designed to provide the highest degree of quality care and personalized services tailored to each resident’s Individualized Service Plan (ISP). The Care Manager is directly involved in hands-on care and supports residents' physical and emotional needs, maintaining a safe and comfortable environment that feels like home. This position requires adherence to Sunrise’s mission, which is to champion quality of life for all seniors, while complying with federal, state or provincial, and local laws as well as Sunrise’s internal standards, policies, and procedures.

In this role, the Care Manager regularly reviews and updates care plans, monitors health changes, communicates with the care team and families, and ensures residents’ dignity and independence are preserved. Additionally, the Care Manager participates in daily operational activities such as housekeeping, laundry, dining service, and risk management to uphold high standards of cleanliness, safety, and wellness. The position also entails engaging residents in life-enriching activities, especially in reminiscence neighborhoods, where multi-sensory experiences and personal histories play a key role in care.

Team collaboration, training, and continuous learning are vital components of this role. Job Requirements

  • High school diploma or GED
  • CPR certificate and First Aid certification as required by law
  • must be at least 18 years old
  • previous senior care experience preferred
  • ability to communicate effectively
  • physical ability to assist with daily living activities
  • willingness to follow all safety and hygiene protocols
  • ability to work collaboratively as part of a team
  • pass background check and drug screening
  • comply with vaccination requirements as applicable

Job Qualifications

Job Duties

  • Review, read, notate, and initial Daily Log to document and learn about pertinent resident information
  • participate in the development of Individualized Service Plans (ISPs) and monthly updates
  • review designated assignments
  • be responsible for a designated group of residents during shift and perform regular physical checks
  • observe, report, and document changes in residents' conditions
  • attend daily Cross Over meetings
  • notify supervisor of increased care needs or condition changes
  • respond promptly to security system and resident call bells
  • greet guests, family members, residents, and team members
  • answer and direct phone calls professionally and timely
  • assist with continence management and proper disposal
  • communicate with families as needed
  • help residents maintain independence, safety, and dignity
  • participate in and assist residents with activities of daily living
  • engage residents in life skills and enrichment activities
  • maintain a warm, calm, and positive atmosphere
  • ensure safety regulations are followed
  • practice good hygiene and cleanliness
  • host and engage in resident activities daily
  • partner with team to ensure compliance with occupational health and safety regulations
  • practice safety procedures including use of PPE, fire extinguishers, and lockout/tagout
  • report all accidents, unsafe conditions, and occupational exposures
  • ensure safe storage and monitoring of oxygen tanks
  • maintain and clean residents' rooms and common areas
  • wash and fold laundry and linens as assigned
  • serve meals and assist in dining room setup and cleanup
  • observe residents' dining habits and document changes
  • provide room service as needed
  • incorporate 'Creating Pleasant Days' concept into reminiscence care
  • blend multi-sensory experiences
  • participate in letter writing with residents and families
  • engage in team meetings and training
  • comply with infection control and training requirements
  • perform other assigned duties as needed
