The role of Care Manager or Designated Care Manager at Sunrise Senior Living is pivotal in delivering exceptional care and services to residents and their families. This position requires a dedicated individual responsible for providing hands-on assistance and emotional support aligned with each resident's Individualized Service Plan (ISP) within assisted living, long-term care, and reminiscence neighborhoods. The Care Manager is not only a caregiver but also an advocate for residents, ensuring safety, comfort, and quality of life are maintained at all times. This role emphasizes fostering meaningful relationships with residents, families, and fellow team members, thereby enhancing the overall community spirit and engagement.
The Care Manager acts as the frontline caregiver accountable for monitoring residents’ conditions continuously, documenting changes, and responding promptly to care needs. This includes participation in daily meetings, coordinating care plans, and proactively communicating changes to supervisors and residents' families to maintain seamless care delivery. Job Requirements
- must be at least 18 years of age
- ability to react and remain calm in difficult situations
- ability to handle multiple priorities
- must complete and pass a drug test and tuberculosis test
- must pass physical evaluation and background check where required
- willingness to receive COVID-19 and influenza vaccinations if mandated
- understand and comply with safety procedures and infection control measures
- ability to lift and assist residents as required
Job Qualifications
- high school diploma or GED accepted
- CPR certificate and first aid certification as required by state or provincial regulations
- previous experience working with seniors preferred
- desire to serve and care for seniors
- ability to make choices and decisions in the resident's best interest
- good written and verbal communication skills
- competent organizational and time management skills
- good judgment, problem solving and decision-making skills
Job Duties
- review, read, notate, and initial daily log to document and learn about pertinent information about residents
- participate in the development of individualized service plans and monthly updates
- review designated assignments
- responsible for a designated group of residents during the shift and physically check on them throughout the shift
- observe, report, and document symptoms and conditions of residents for changes in condition
- attend daily cross over meetings
- notify supervisor and resident care director if a resident has increased care needs
- respond to security system and call bells promptly and take appropriate action
- assist with continence management and sanitation
- communicate with families and serve as a resource
- help residents maintain independence, safety, and dignity
- participate in activities of daily living and encourage participation
- engage residents in life skills and enrichment activities
- practice standard care precautions and safety regulations
- maintain cleanliness and perform housekeeping duties
- serve and support dining services, ensuring pleasant dining experience
- incorporate reminiscence care practices and multi-sensory activities for residents
- participate as a team member, uphold team credo and principles of service
- attend training sessions and meetings
- comply with infection control and safety protocols
- report accidents, incidents, and hazards immediately
- perform other duties as assigned
