The Care Manager role at Sunrise of Issaquah is pivotal in delivering personalized care to residents in assisted living, long-term care, and reminiscence neighborhoods. Job Requirements
- High school diploma or GED
- CPR certificate and First Aid certification per state or provincial regulations
- Minimum age of 18 years
- Previous experience working with seniors preferred
- Desire and commitment to serve and care for seniors
- Ability to make appropriate decisions in the resident's best interest
- Passing drug test and tuberculosis test
- Background check clearance
- Physical evaluation clearance
- Covid-19 and influenza vaccination if mandated
- Compliance with Sunrise standards and regulatory requirements
- Ability to attend regular training and team meetings
- For those driving Sunrise vehicles, signing a Driver job description and understanding all safety duties.
Job Qualifications
- High school diploma or GED accepted
- CPR certificate and First Aid as required by regulations

- As applicable, understanding of driving responsibilities for Sunrise vehicles.
Job Duties
- Review, read, notate, and initial Daily Log to document and learn about pertinent resident information
- Participate in development and monthly updates of Individualized Service Plans (ISP)
- Maintain responsibility for a designated group of residents during shifts, including physical checks
- Observe, report, and document changes in residents' symptoms and conditions
- Respond promptly and appropriately to security system and resident call bells
- Greet and assist guests, family members, residents, and team members courteously
- Assist with continence management ensuring sanitation
- Help residents maintain independence and safety adhering to Resident Rights and service standards
- Lead and assist residents with daily living activities and life enrichment activities
- Partner with community team to ensure compliance with occupational health and safety regulations
- Practice safety procedures including use of personal protective equipment and hazardous materials handling
- Report accidents, unsafe conditions, and occupational exposures promptly
- Maintain cleanliness and sanitation in resident rooms and common areas
- Serve meals and assist in dining room setup and clean up
- Follow residents' dietary requirements and observe dining habit changes
- Provide room service delivery as needed
- Engage residents in reminiscence neighborhood with multi-sensory activities and social events
- Participate in team training sessions and contribute to team goals
- Comply with infection control and cleaning protocols
- Perform other duties as assigned.
