The Care Facilitator role at ChenMed is a pivotal position serving as one of the first points of contact between the patient and the ChenMed family. This role is designed for individuals with a strong commitment to superior customer service who can act as effective patient advocates. The Care Facilitator plays an essential administrative and secretarial role, responsible for checking patients in and out of the medical clinic, supporting scheduling duties, and maintaining the organization of patient information and clinic operations. They are expected to build trusting relationships with patients through exceptional service and effective communication.

The Care Facilitator’s duties are diverse and meaningful, including greeting patients, managing appointment schedules for primary care physicians (PCPs) and specialists, verifying patient insurance, and ensuring accurate documentation in electronic medical records (EMR) systems. This role also involves proactive patient engagement tasks such as conducting "love" calls and sending birthday and greeting cards to commemorate special events, enhancing the personalized care experience. The Care Facilitator manages logistical aspects like transportation coordination for patients, including ridesharing services and medication deliveries while supporting clinical operations by providing visibility for HEDIS needs and hospitalizations.

Applicants for this role should have a high school diploma or equivalent, with a preference for graduates of nationally accredited Medical Assistant programs. Previous experience of at least one year in a medical clinic or a similar environment is required, and basic knowledge of medical insurance and EMR systems is essential. The ability to operate office technology and software, strong communication skills, attention to detail, professionalism, and a positive attitude are important qualities. Flexibility to work evenings, weekends, or holidays and willingness to travel locally up to 10% is needed.

Compensation for this hourly position ranges from $14.30 to $20.42, depending on experience and geographic location. In addition to base pay, employees may be eligible for bonuses or commissions. Job Requirements

  • minimum of 1 year of work experience in a medical clinic or similar environment

Job Qualifications

Job Duties

  • Interacts appropriately with patients and records information while greeting, checking-in, checking-out or scheduling appointments for patients
  • conducts lobby checks to ensure organization and cleanliness
  • serves as the gatekeeper for PCP and specialist schedules including ensuring top 40 patients are scheduled every 13 days
  • utilizes computer equipment and programs to add new patient information, print relevant forms, schedule appointments, respond to emails and generate new patient packets
  • conducts "love" calls and mails birthday and greeting cards to patients
  • answers telephones, takes detailed and accurate messages, and directs messages appropriately
  • communicates information about ChenMed clinical personnel to patients
  • verifies patient insurance and collects necessary copays
  • assists with scheduling follow-up and in-house specialist referral appointments
  • escorts patients to exam rooms and ensures their comfort
  • acts as HEDIS champion for the PCP by providing visibility of HEDIS needs for scheduled patients
  • reports on hospitalizations including HITS census and ENS Ping
  • presents patients with customer service surveys during check out and escalates for service recovery if needed
  • participates in daily and weekly huddles to provide details on patients
  • regulates and monitors transportation including driver’s lists, medication and grocery deliveries, driver behavior, and coordinating Uber/Lyft rides for patients
  • maintains confidentiality of patients’ personal information
  • reviews patient appointment information for completeness and accuracy
  • transmits correspondence by mail, email or fax
  • performs other duties as assigned.
