The role available at Frontier Behavioral Health is for a Care Coordinator position, an essential component of the behavioral health care continuum. The Care Coordinator is responsible for managing the care of individuals who face multiple chronic conditions and are medically vulnerable, often at risk for frequent emergency care usage. This position supports members by assessing their needs, facilitating education and support, coordinating referrals to community and clinical resources, and helping them develop and maintain personalized Health Action Plans. With a primary focus on improving clients’ self-management and health outcomes, the Care Coordinator operates mainly in community settings and often meets clients in their homes. This role demands compassion, excellent organizational skills, and the ability to navigate diverse environments as the coordinator works closely with clients to address medical, social, and behavioral health issues. Job Requirements

  • Bachelor's degree in behavioral sciences
  • One year related experience in behavioral health or health care setting
  • Ability to apply for Counselor Agency Affiliated - Registered credential within 30 days of hire
  • Valid driver's license
  • Active car insurance
  • Willingness to use personal vehicle in course of work

Job Qualifications

  • Bachelor's degree in behavioral sciences or related field
  • One year of related experience in behavioral health care or health care setting
  • Ability to apply for Counselor Agency Affiliated - Registered credential within 30 days of hire
  • Valid driver's license active car insurance and willingness to use personal vehicle for work
  • Experience working with children is preferred but not required

Job Duties

  • Meet in person and over the phone with assigned members to initiate develop and finalize Health Action Plans
  • Screen assess and provide brief interventions for common mental and substance abuse disorders
  • Complete required periodic health and behavioral screenings
  • Use decision support tools and registries to coordinate services and care
  • Monitor members for changes in symptoms and treatment side effects linking with care providers
  • Provide brief intervention using behavioral activation motivational interviewing or related techniques
  • Document patient progress treatment recommendations and revise Health Action Plans
  • Facilitate referrals for social and specialty services such as housing assistance vocational rehabilitation mental health and substance abuse treatment
  • Complete core training curriculum and participate in ongoing training and technical assistance
  • Comply with contract requirements and agency policies
  • Maintain client records and documentation confidentially and timely
  • Travel to see clients in community and between work sites
  • Perform other duties as assigned
