The Care Coordinator & Navigation Specialist role in the Health Homes Program is a pivotal position within Sunrise Services. This program specifically targets high-risk clients, including pediatric populations, who face significant challenges such as chronic health conditions, mental health issues, substance use disorders, or long-term care requirements. The primary function of the Health Homes Program is to ensure these clients receive coordinated, holistic care by integrating clinical expertise with social support services. As a Care Coordinator & Navigation Specialist, you will play an essential role in facilitating access to appropriate services, supporting engagement efforts, and assisting clients in navigating complex healthcare systems. This role involves working closely with a multidisciplinary team of health professionals to ensure continuity and quality of care.

Responsibilities include managing referrals from regional councils and healthcare entities, assisting in screening and client assignment processes, and engaging clients through various communications to secure and confirm appointments. The position requires interpersonal skills to build client rapport, especially with hard-to-reach populations, as well as organizational abilities to support program logistics, such as preparing for site visits and audits. Sunrise offers a competitive hourly wage of $22 to $25.50, dependent on experience, education, and licensure, alongside a comprehensive benefits package that underscores the organization’s commitment to employee well-being.

Additionally, Sunrise Services emphasizes compliance with licensing and certification standards, including maintaining valid driver’s licenses and first aid certifications. The working hours are Monday through Friday, 8 am to 4:30 pm, supporting a healthy work-life balance. Job Requirements

  • Must meet one of the specified education and experience criteria
  • current valid Washington driver’s license with acceptable driving record
  • $50,000 automobile liability insurance required
  • first aid and CPR certification
  • licensure related to qualifications listed above
  • DSHS background clearance including fingerprint
  • acceptable driving record
  • strong verbal and written communication skills
  • skilled with Microsoft Office Suite
  • ability to lift approximately 20-30 pounds and physically assist unsteady clients

Job Qualifications

  • Bachelors level degree in related mental health field with 2 years’ experience
  • behavioral Health Professional or Specialist - Mental Health Counsellor with Masters level degree in related field
  • certified MH Counselor with Bachelors level degree in related field
  • Agency Affiliated Counsellor with Bachelors level degree in psychology
  • registered Nurse, Nurse Practitioner, Licensed Practical Nurse
  • Social Worker – Masters or Bachelors in social work, human services or related field
  • Substance Use Disorder Professional

Job Duties

  • Work as a member of a multidisciplinary Health Homes Team
  • assist Program Coordinator to process referrals received from Lead Entities
  • assist Program Coordinator and Care Coordinators in screening and generating referrals
  • assist Program Coordinator in ‘smart assignment’ of clients to Care Coordinators
  • assist in engaging hard to reach clients, establishing rapport and making arrangements for Care Coordinators to have in person contacts
  • carry out pre-engagement activities with clients and schedule face to face appointments for Care Coordinators in the field
  • engage with clients via telephone and mail out activities to secure and confirm appointments with Care Coordinators
  • assist colleagues to obtain necessary consents and assist in identifying initial client objectives and goals identified at referral and during initial telephone outreach
  • assist Program Coordinator in preparation for site visits and audits
