Job Description
Casa de la Familia is a dedicated non-profit organization committed to providing comprehensive mental health services to individuals and families impacted by trauma, anxiety, depression, and various behavioral health conditions. Located in Southern California, the organization collaborates closely with healthcare providers, social service agencies, educational institutions, community organizations, and public agencies to improve access to healthcare and support for underserved and diverse populations. Casa de la Familia places a strong emphasis on culturally responsive care, ensuring that all individuals, regardless of background, receive compassionate and effective mental health services tailored to their unique needs and circumstances.
The organization’s mission centers on fostering wellness and resilience in the community through accessible, high-quality mental health care and social support. Casa de la Familia’s approach is holistic, recognizing that mental health is intricately connected to social determinants such as housing, food security, transportation, and community support systems. By addressing these interconnected factors, Casa de la Familia helps individuals and families build healthier, more stable lives.
The Bilingual Care Coordinator position at Casa de la Familia is an integral role within the Enhanced Care Management (ECM) program, funded through CalOptima, aimed at assisting members who require coordinated care across multiple service areas. This role acts as a crucial point of contact for ECM members, providing personalized outreach, engagement, appointment scheduling, resource connection, and detailed documentation. The Care Coordinator ensures that members can navigate complex healthcare and social service systems, linking them to medical care, behavioral health services, housing, food assistance, transportation, and other vital supports.
This position is perfectly suited for dedicated individuals who find fulfillment in helping others, enjoy engaging with people primarily via phone, excel at problem-solving, and are passionate about improving health outcomes and quality of life for vulnerable populations. The Care Coordinator plays a key role in fostering strong partnerships with healthcare providers, internal care teams, and community resources, ensuring that every member receives timely and coordinated support.
