The Care Concierge role at Curana Health is a vital position within the Interdisciplinary Care Team. Located on-site at senior living facilities, the Care Concierge acts as a critical link between patients, care providers, and Curana’s virtual support team. This role is centered around ensuring smooth and effective communication and coordination to meet the complex and evolving needs of seniors enrolled in programs such as Advanced Primary Care Management (APCM) and Guiding an Improved Dementia Experience (GUIDE).

Care Concierges perform monthly patient rounds, respond to inquiries from patients and their durable power of attorneys, manage electronic health records, coordinate appointments and necessary documentation with providers, and assist in organizing telehealth consultations. The position demands a diligent and compassionate individual who can interpret medical information accurately, escalate clinical findings appropriately, and provide logistical and emotional support to patients and caregivers alike. Additionally, the role involves collaboration across Curana’s interdisciplinary care teams to ensure comprehensive and continuous support for senior living residents.

Given the nature of the work, the Care Concierge travels frequently to local senior living communities, spending approximately four business days per week on-site. This travel component requires candidates to have flexibility, reliability, and, potentially, a valid driver’s license and driving record depending on future company policy. Job Requirements

  • High school diploma or equivalent
  • 1 plus years of experience working in a medical office, community engagement, or related fields
  • Ability to interpret medical records, lab results, and appointment notes
  • Experience in collecting clinical information and surfacing changes to the appropriate care team member
  • Willingness to travel to local Senior Living Communities
  • Proficient computer skills and knowledge of Microsoft Office applications
  • Excellent organizational and time management skills
  • Ability to prioritize tasks
  • On-site presence at assigned Senior Living Communities 4 business days per week
  • May require travel to multiple local Senior Living Communities

Job Qualifications

  • High school diploma or equivalent
  • 1 plus years of experience working in a medical office, community engagement, or other related fields
  • Ability to interpret medical records, lab results, and appointment notes
  • Experience in collecting clinical information and surfacing changes to the appropriate Curana care team member
  • Willingness to travel to local Senior Living Communities
  • Proficient computer skills and knowledge in using various Microsoft Office applications
  • Excellent organizational and time management skills with the ability to prioritize tasks
  • Experience working with chronic care management or other care management programs preferred

Job Duties

  • Conduct monthly in-person rounds for patients enrolled in Advanced Primary Care Management or the Guiding an Improved Dementia Experience programs
  • Address patient and durable power of attorney inquiries via telephone
  • Respond to patient or caregiver messages received via the Curana Patient Portal
  • Assist patients with scheduling follow-up appointments with Curana Providers or specialists
  • Manage electronic health records and ensure accurate and up-to-date patient records
  • Coordinate documents needed for review or signature by a provider
  • Facilitate provider orders and escalate findings
  • Support Provider scheduling
  • Assist with prior authorizations
  • Obtain patient records and diagnostic test results
  • Organize telehealth consultations
  • Collaborate with the Curana Interdisciplinary Care Team to ensure seamless communication within our health network
  • Perform other duties as assigned
