The Care Concierge role at Curana Health is a vital component of the company’s Interdisciplinary Care Team. Serving as the on-site liaison at senior living facilities, the Care Concierge acts as a critical bridge between patients, healthcare providers, and Curana's virtual support team, ensuring seamless communication and coordination of care. This position offers a unique opportunity for individuals passionate about senior care and healthcare coordination to make a meaningful, tangible impact on the lives of older adults by supporting complex care programs such as Advanced Primary Care Management (APCM) and Guiding an Improved Dementia Experience (GUIDE).
The Care Concierge is responsible for a variety of essential tasks including conducting regular patient rounds, assisting patients and their durable power of attorney with inquiries, facilitating follow-up appointment scheduling, and managing electronic health records with precision to uphold quality standards. Additional responsibilities include assisting providers with document coordination, supporting telehealth consultations, handling prior authorizations, and facilitating communication across Curana’s interdisciplinary team.
Ideal candidates for this role have at least one year of experience in a medical office, community engagement, or related healthcare field, demonstrate the ability to interpret complex medical information such as lab results and appointment notes, and possess strong organizational and computer skills. The role requires onsite presence at senior living communities four business days per week with potential travel to multiple local facilities. Job Requirements
- High school diploma or equivalent
- Minimum one year experience in medical office, community engagement, or related fields
- Ability to interpret medical and clinical information
- Willingness to travel to assigned senior living communities for four business days per week
- Proficiency in computer skills including Microsoft Office
- Strong organizational and time management skills
- Ability to communicate effectively with patients, families, and interdisciplinary teams
Job Qualifications
- High school diploma or equivalent
- At least one year of experience working in medical office, community engagement, or related healthcare fields
- Ability to interpret medical records, lab results, and appointment notes
- Experience in collecting clinical information and effectively communicating changes to care team members
- Willingness to travel to local senior living communities
- Proficient computer skills including Microsoft Office applications
- Excellent organizational and time management skills
- Experience working with chronic care management or other care management programs preferred
Job Duties
- Conduct monthly in-person rounds for patients enrolled in Advanced Primary Care Management or the Guiding an Improved Dementia Experience programs
- Address patient and durable power of attorney inquiries via telephone
- Respond to patient or caregiver messages received via the Curana Patient Portal
- Assist patients with scheduling follow-up appointments with Curana providers or specialists
- Manage electronic health records and ensure accurate and up-to-date patient records
- Coordinate documents needed for review or signature by a provider
- Facilitate provider orders and escalate findings
- Support provider scheduling
- Assist with prior authorizations
- Obtain patient records and diagnostic test results
- Organize telehealth consultations
- Collaborate with the Curana Interdisciplinary Care Team to ensure seamless communication within the health network
- Perform other duties as assigned
