The Care Concierge role at Curana Health is a vital position within the Interdisciplinary Care Team, offering direct support on-site at senior living facilities. This role acts as a critical communication bridge connecting patients, healthcare providers, and Curana’s virtual support teams to ensure seamless care delivery. The Care Concierge is responsible for regularly engaging with patients enrolled in programs such as Advanced Primary Care Management (APCM) and Guiding an Improved Dementia Experience (GUIDE), providing essential patient support by conducting monthly in-person patient rounds, addressing inquiries, and assisting with appointment scheduling. Additionally, the role involves comprehensive provider support tasks, including managing electronic health records, coordinating provider documentation, securing prior authorizations, obtaining patient records, and facilitating telehealth consultations. The Care Concierge also plays an important part in maintaining clear and efficient communication across Curana’s interdisciplinary care network. This position requires a hands-on, client-focused individual with strong organizational abilities, exceptional interpersonal skills, and proficiency in medical and administrative tasks. Frequent travel to assigned senior living communities is expected, reflecting the on-site nature of this role. Job Requirements

  • High school diploma or equivalent
  • Minimum 1 year of experience in a medical office, community engagement, or related field
  • Ability to interpret medical records, lab results, and appointment notes
  • Proficiency in Microsoft Office applications
  • Strong organizational and time management skills
  • Ability and willingness to travel locally to senior living communities
  • Ability to communicate effectively with patients, caregivers, and providers

Job Qualifications

  • High school diploma or equivalent
  • 1+ years of experience working in a medical office, community engagement, or related fields
  • Ability to interpret medical records, lab results, and appointment notes
  • Experience in collecting clinical information and surfacing changes to care team members
  • Proficient computer skills with knowledge of Microsoft Office applications
  • Excellent organizational and time management skills
  • Willingness to travel to local senior living communities
  • Experience with chronic care management or other care management programs preferred

Job Duties

  • Conduct monthly in-person rounds for patients enrolled in APCM or GUIDE programs
  • Address patient and durable power of attorney inquiries via telephone
  • Respond to patient or caregiver messages received via the Curana Patient Portal
  • Assist patients with scheduling follow-up appointments with Curana Providers or specialists
  • Manage electronic health records and ensure accurate and up-to-date patient records
  • Coordinate documents needed for review or signature by a provider
  • Facilitate provider orders and escalate findings
  • Support provider scheduling
  • Assist with prior authorizations
  • Obtain patient records and diagnostic test results
  • Organize telehealth consultations
  • Collaborate with the Curana Interdisciplinary Care Team to ensure seamless communication
  • Perform other duties as assigned
