The Care Concierge position at Curana Health is a vital role within the company's Interdisciplinary Care Team. This role is primarily on-site at senior living facilities, acting as the crucial link between patients, their providers, and Curana's virtual support staff. The Care Concierge is responsible for supporting patients enrolled in specialized healthcare programs like Advanced Primary Care Management (APCM) and Guiding an Improved Dementia Experience (GUIDE) by conducting monthly in-person rounds and responding promptly to patient and durable power of attorney inquiries. This position requires strong communication skills to address patient or caregiver messages received via the Curana Patient Portal and assist in scheduling follow-up appointments with Curana Providers or specialists.

Additionally, the Care Concierge manages electronic health records (EHR) to ensure accuracy and timeliness of patient data, coordinates documentation for provider review or signatures, facilitates provider orders, and supports provider scheduling and prior authorizations. They also organize telehealth consultations and assist in obtaining necessary patient records or diagnostic results, ensuring seamless healthcare delivery. Effective communication and collaboration with the wider Curana Interdisciplinary Care Team are essential components of this role, promoting integrated care across the health network.

The position requires travel to local senior living communities, spending approximately four business days per week on-site at assigned facilities. Curana Health values candidates with a high school diploma or equivalent, preferably registered or certified medical assistants with at least one year of experience in medical offices or community engagement. Strong skills in interpreting medical records and lab results, proficient computer literacy with knowledge of Microsoft Office applications, organizational prowess, and time management capabilities are critical. Experience in chronic care management or other care management programs is desirable.

Job Requirements

Job Duties

  • Conduct monthly in-person rounds for patients enrolled in Advanced Primary Care Management (APCM) or the Guiding an Improved Dementia Experience (GUIDE) programs

  • Respond to patient or caregiver messages received via the Curana Patient Portal
