The role of Care Concierge is an integral part of Curana's Interdisciplinary Care Team, acting as a crucial liaison between patients, healthcare providers, and Curana's virtual support team. The Care Concierge works on-site at senior living communities, ensuring effective communication and coordination among all parties to enhance patient care management. This position offers a competitive pay range of USD $20.00 to $24.00 per hour, subject to factors such as education, experience, and certifications. Additional benefits include a comprehensive benefits package, 401K, paid time off (PTO), and paid holidays, all contingent on eligibility.
In this role, you will be responsible for conducting patient rounds, responding to inquiries, assisting with scheduling, managing electronic health records, supporting provider needs such as prior authorizations, and facilitating telehealth consultations. You will collaborate closely with the interdisciplinary team to uphold seamless communications and high-quality care delivery. The position requires strong organizational skills, proficiency with Microsoft Office applications, and a willingness to travel locally to assigned senior living communities at least four business days per week. Job Requirements
- Registered or certified medical assistant
- One or more years of experience working in a medical office, community engagement, or related fields
- Willingness to travel to assigned senior living communities four business days per week
- Proficient in use of Microsoft Office applications
- Excellent organizational and time management skills
- Ability to interpret medical records, lab results, and appointment notes
- Ability to collect clinical information and escalate findings appropriately
Job Qualifications

- One or more years of experience working in a medical office, community engagement, or related fields

- Experience in collecting clinical information and surfacing changes to the appropriate care team member
- Willingness to travel to local senior living communities
- Proficient computer skills and knowledge of Microsoft Office applications
- Excellent organizational and time management skills with ability to prioritize tasks
- Experience with chronic care management or other care management programs (preferred)
Job Duties
- Conduct monthly in-person rounds for patients enrolled in Advanced Primary Care Management or the Guiding an Improved Dementia Experience programs
- Address patient and durable power of attorney inquiries via telephone
- Respond to patient or caregiver messages received via the Curana Patient Portal
- Assist patients with scheduling follow-up appointments with Curana providers or specialists
- Manage electronic health records and ensure accurate and up-to-date patient records
- Coordinate documents needed for review or signature by a provider
- Facilitate provider orders and escalate findings
- Support provider scheduling
- Assist with prior authorizations
- Obtain patient records and diagnostic test results
- Organize telehealth consultations
- Collaborate with the Curana Interdisciplinary Care Team to ensure seamless communication within the health network
- Perform other duties as assigned
