The Devoted Medical Transitions of Care (TOC) Advanced Practice Provider (APP) role is a critical position designed to support patients during the vulnerable 30-day period following hospital discharge. This full-time, remote role offers a structured 5-day workweek with consistent hours within CST, MST, or PST time zones from 8:30 AM to 5:00 PM. Salary for this position ranges from $120,000 to $150,000 annually, with an additional performance-based bonus ranging from $10,000 to $20,000, bringing total compensation to between $130,000 and $170,000 per year. The TOC APP position encompasses roles for both Nurse Practitioners (APRNs) and Physician Assistants (PAs), making it an opportunity for experienced providers to be part of a pioneering program designed to prevent hospital readmissions. In this role, approximately 70% of the provider's time will be devoted to comprehensive initial post-discharge visits, including thorough assessment of medical conditions, medication reconciliation, and care planning. The remaining 30% focuses on follow-up visits and urgent symptom management, requiring acute clinical judgment to manage complex, decompensating patients effectively.

Candidates must be comfortable navigating the nuances of chronic disease management alongside acute symptom intervention, delivering whole-person care that adapts over time. This dual skill set—balancing immediate clinical decisions and long-term care planning—is essential for success. The role also involves active participation in interdisciplinary rounds featuring care coordinators, nurse case managers, social workers, pharmacists, and physicians to collaboratively reduce readmission risks and optimize patient outcomes.

Beyond clinical responsibilities, this position actively contributes to the growth and evolution of the TOC program. Providers are expected to help shape team culture, promote open communication, engage in continuous quality improvement initiatives, and provide valuable input to enhance the homegrown electronic health record system. Job Requirements

  • Active and in-good-standing RN and APRN licenses for Nurse Practitioners
  • Active and in-good-standing Physician Assistant license for Physician Assistants
  • Licensure and good standing in required Devoted states with willingness to obtain additional state licenses within 60-90 days
  • Licensure in FL, TX, AZ, OH, or GA required to start
  • Experience with managing acute and chronic disease exacerbations
  • Ability to work Monday-Friday 8:30 AM - 5:00 PM within CST, MST, or PST time zones
  • Proficiency with telehealth video platforms
  • Strong clinical judgment with bias to action in virtual settings
  • Experience working with frail and vulnerable populations including skilled nursing facility settings
  • Ability to adapt in a fast-paced, changing environment

Job Qualifications

  • 5+ years of direct patient care managing geriatric populations with multiple chronic complex conditions
  • Demonstrated ability in both acute management and longitudinal chronic disease care
  • Strong, independent clinical decision-making skills
  • Proficiency in complex medication reconciliation and guideline-directed medical therapy
  • Experience diagnosing and managing decompensating patients with heart failure, COPD, diabetes, hypertension, pneumonia, cellulitis, and urinary tract infections
  • Ability to work full-time 5-day, 8-hour schedule within CST, MST, or PST hours
  • Collaborative team player with interdisciplinary team experience
  • Proficiency in English and Spanish

Job Duties

  • Perform comprehensive initial post-discharge visits including medical assessment, medication reconciliation, and care planning
  • Conduct acute virtual urgent-care visits to assess, diagnose, and manage new or worsening symptoms
  • Manage patients' clinical trajectory and adjust care plans over a 30-day period
  • Participate in interdisciplinary TOC rounds collaborating with care coordinators, nurse case managers, social workers, pharmacists, and physicians
  • Enhance team culture by facilitating open communication and supporting patient escalations
  • Assist with clinical education of the healthcare team
  • Participate in quality improvement projects and provide feedback to improve electronic health record systems
