The Lead Care Coordinator (LCC) position at Victor plays a crucial role within the Enhanced Care Management (ECM) Program, operating under the supervision of the Director of Innovation. This role requires a highly committed professional who will provide direct client services and coordinate complex care management as a vital part of a care team. The LCC oversees specific client cases, ensures coordination of health care benefits, facilitates education, and enables access to timely and cost-effective care services. Collaboration and communication with clients, family supports, caregivers, and a network of health and social service providers are paramount to promote client wellness, recovery, independence, resilience, and empowerment.
The LCC acts as an advocate for their clients and serves as an active member within interdisciplinary teams. This role requires managing linkages with hospitals, social service programs, and external providers while coordinating transportation and office visits as necessary. The position demands thorough documentation practices, ongoing client progression evaluations, and participation in regular staff meetings and professional training opportunities such as ACEs Certification.
Ideal candidates will possess a combination of practical experience and educational qualifications, demonstrating bilingual proficiency in English and Spanish, excellent communication, and customer relations skills along with PC hardware/software familiarity. This full-time role offers a competitive pay range of $24.16 to $33.23 per hour, with additional pay incentives for educational achievements and bilingual skills. Job Requirements
- Must reside in Riverside or San Bernardino County
- Must possess valid California driver’s license and personal automobile insurance
- Maintain driving record meeting Agency’s Motor Vehicle Operating Standards
- Must be physically and mentally fit per Agency’s Physical Fitness Standards
- Willingness to complete tuberculosis (TB) test and drug screening
- Consent to personal background investigation by State of California
- Willingness to work in culturally integrated environment respecting differences based on protected classes
- Ability to perform physical tasks including standing, walking on uneven surfaces, bending, kneeling, twisting, reaching, balancing, carrying up to 25 pounds occasionally and 10 pounds frequently
- Ability to routinely walk stairs and sit for long periods in vehicle up to 100 miles
- Ability to operate computer and telephone systems requiring fine hand manipulation for typing and filing
Job Qualifications
- High school diploma or equivalent
- Three years experience in health, education, social services or related field OR one year experience with associate degree OR one year experience plus two years lived and/or community engagement experience
- Bilingual in English and Spanish
- Excellent customer and employee relations skills
- Superior communication skills including writing, editing, vocabulary, grammar, spelling, and punctuation
- Proficiency with PC hardware/software and Microsoft Word and Excel
- Ability to complete assigned training and ongoing educational requirements

- Conduct outreach and engagement via calls, mail, and field visits to link clients to ECM program
- Arrange transportation and accompany clients to appointments when necessary
- Evaluate client progress and update goals
- Complete required documentation timely
- Participate in weekly staff/team meetings and supervision
- Attend assigned training including ACEs Certification
- Perform additional duties as assigned
