The Child and Youth Wraparound Case Manager role is an intensive, individualized case management position aimed at supporting children and families dealing with complex behavioral health challenges. Using the National Wraparound Initiative (NWI) model, this position plays a critical role in preventing out-of-home placements by assembling and leading a team of professionals and natural supports—including family members and friends—to develop and implement a single, unified plan of care tailored to the unique needs of each family. The Case Manager will handle a manageable caseload of approximately 10 families, enabling a high degree of personalized attention and support.

Ideal candidates will possess a deep understanding of child and youth mental health, particularly within community settings, and will thrive in dynamic, fast-paced environments. Frequent in-home and community-based visits are part of the core responsibilities, requiring strong interpersonal skills and resilience in high-stress situations. The role operates primarily out of the Conroe office in Montgomery County, with occasional travel to serve families in Walker and Liberty counties, including Spanish-speaking communities. The schedule typically runs Monday through Thursday from 8:00 a.m. to 7:00 p.m., with Fridays off, promoting a healthy work-life balance. Mileage reimbursement is provided for work-related travel.

Compensation for this position is competitive, with an hourly rate of $26.21, equating to an annual salary of approximately $54,525.
