This specific role is an Analyst Case Management position within Aetna's Long-Term Services and Supports (LTSS) team. This is a full-time, field-based position serving locations including North Miami, Miramar, Aventura, Hialeah, and Flagler in Florida. Candidates must be willing to travel extensively—50 to 75 percent of the time—to engage with members face to face, ensuring that their medical and supportive care needs are managed promptly and effectively. The standard working hours for this position are Monday through Friday from 8:00 a.m. to 5:00 p.m., reflecting a structured schedule conducive to balancing personal and professional responsibilities.

The Case Management Coordinator plays a vital role in coordinating, planning, and implementing case management activities to support members' overall wellness. This role requires a tech-savvy, self-motivated, energetic individual who is detail-oriented and highly organized. The position offers a competitive salary range from $21.10 to $40.90 per hour, depending on factors such as experience, education, and location. Additionally, the position includes a $5,000 new hire bonus, enhancing the reward for qualified applicants. The Case Management Coordinator will use effective communication skills in both English and Spanish, leveraging bilingual abilities to better serve diverse populations within the Florida service area. This role suits professionals with experience in behavioral health or long-term care, who are adept at multitasking and are flexible in adapting to the fast-paced healthcare environment. Candidates should reside in designated Florida locations and have reliable transportation for required travel, for which mileage reimbursement is provided.

Job Requirements

  • Must reside in North Miami, Pembroke, Miramar, Aventura, Hialeah, or Flagler FL
  • must possess reliable transportation and able to travel 50-75% of the time
  • 1+ year of relevant experience in behavioral health or long-term care
  • bi-lingual in English and Spanish
  • computer proficiency in Microsoft Word, Excel, and Outlook
  • ability to work Monday to Friday, 8:00am to 5:00pm
  • must be self-motivated and highly organized

Job Qualifications

  • Bachelor's degree preferably in social work or related field
  • 1+ year of experience in behavioral health or long-term care setting
  • bi-lingual in English and Spanish
