In this supportive role, you will be involved in a wide range of daily activities ranging from personal care and socialization to community integration and transportation. You will collaborate closely with clinical staff to ensure therapeutic and behavioral plans are followed effectively. Documentation of progress and service changes is essential, as is providing clinical expertise in areas relevant to the unique needs of the individuals served. Your role may also include accompanying individuals to medical appointments and acting as a bridge between medical providers and clients to relay important information. With travel between sites and homes, you will help facilitate community participation and enhance the quality of life for those under your care.
