The Behavioral Health Advocate position, based in Lucas County, Ohio, is a dynamic role designed for individuals passionate about mental health and community well-being. This role combines direct member interaction with the utilization of technology and community resources to facilitate smooth healthcare transitions and improve patient outcomes. The Behavioral Health Advocate primarily supports Medicaid members by conducting home visits and other community-based interactions to ensure continuity of care, provide education on medical conditions, review medications, and connect members to essential services such as Home Health Aides and Meals on Wheels. Approximately 35 percent of the work involves field visits to members’ homes, community centers, hospitals, and provider offices, offering a mix of home-based and remote work flexibility, especially for those located near Toledo, OH.

This position requires a strong ability to communicate and coordinate between healthcare providers and patients, acting as an educator, evaluator, and service coordinator. The role focuses on reducing hospital readmissions, emergency room visits, and service gaps that can significantly impact the wellbeing of vulnerable populations while also easing the strain on the healthcare system. Job Requirements

  • Hold an active, independent, and unrestricted license in the state of Ohio
  • Have 2 or more years of post-licensure mental health related experience
  • Reside in or near Toledo, OH
  • Possess proficient computer skills with knowledge of Microsoft Word and Outlook
  • Have access to high-speed internet and a distraction-free workspace at home
  • Have reliable transportation and ability to travel within the Toledo, OH area
  • Comply with UnitedHealth Group's Telecommuter Policy if working remotely

Job Qualifications

  • Independently licensed mental health clinician with a master’s degree in psychology, social work, counseling, marriage or family counseling, or a related mental health discipline
  • Registered nurse (RN) certified as a psychiatric nurse or mental health clinical nurse specialist by the American Nurses Credentialing Center
  • RN with a master’s degree in nursing specializing in psychiatric/mental health and 2 years of supervised clinical experience
  • Licenses must be active, independent, and unrestricted in Ohio
  • 2+ years of post-licensure experience in a related mental health environment
  • Proficient computer skills with working knowledge of Microsoft Word and Outlook
  • Experience working with Medicaid populations preferred
  • Community mental health experience preferred
  • Experience in benefit coordination and utilization of multiple resources for patients preferred
  • Experience with low-income populations preferred
  • Experience with Medicaid or Medicare funded programs preferred
  • Medical or behavioral health setting experience (hospital, managed care organization, outpatient practices) preferred
  • Dual diagnosis experience with mental health and substance abuse preferred
  • Clinical training experience preferred

Job Duties

  • Visit Medicaid members in their homes and other settings including community centers, hospitals, or providers offices
  • Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction
  • Support members with condition education, medication reviews, and connections to resources such as Home Health Aides or Meals on Wheels
  • Assist members with the transition from a care facility back to their home
  • Act as an intermediary between providers and members to coordinate care
  • Evaluate member needs and develop service plans
  • Conduct community resource research to support member well-being
