Job Requirements

  • Master's or bachelor's degree in social work, occupational therapy, or related healthcare field
  • Minimum of 3 years of work experience with older adults
  • Knowledge of major aging issues including loss, grief, depression, and physical changes
  • Experience interfacing with community resources
  • Strong organizational skills including punctuality and responsiveness
  • Ability to collaborate with peers and caregivers
  • Proficiency with word processing, Microsoft 365 Suite, and other computer software
  • Good driving record and reliable vehicle
  • Professional presentation and ability to maintain confidentiality
  • Effective communication and record-keeping skills
  • Flexible personality and schedule
  • Willingness to work in crisis situations
  • Strong business acumen
  • Ability to maintain effective working relationships

Job Qualifications

  • Master's or bachelor's degree in social work, occupational therapy, or related healthcare field
  • NACCM CMC certification preferred
  • Minimum of 3 years of work experience with older adults
  • Proven experience in business development and marketing in healthcare preferred
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team

  • Assess and monitor clients' health and wellbeing
  • Coordinate personalized care plans with clients and families
  • Advocate for client needs and preferences
  • Provide education and support to family caregivers
  • Collaborate with healthcare professionals and community resources
  • Assist with business development and marketing efforts
  • Maintain accurate client records and documentation
