Job Qualifications

Arizona active RN or LPN licensure
Ability to meet all state and local employment screening requirements including drug, background, and abuse registry screenings

Job Duties

Coordinate and oversee care services delivery to residents
Liaise with hospital personnel, physicians, and community organizations
Conduct initial assessments and develop personalized care plans
Regularly review and update resident care plans
Manage and supervise caregivers and healthcare staff including hiring, training, and evaluation
Ensure proper medication management and adherence
Maintain compliance with local, state, and federal assisted living regulations
Maintain open communication with residents and families
Maintain accurate and compliant documentation of health status and services
Advocate for residents' rights and preferences
Continuously seek opportunities to improve care services and programs
