This specific role is for an Advanced Practice Clinician (APC), either a Nurse Practitioner or Physician Assistant, who will work primarily in a field-based capacity conducting annual comprehensive in-home health assessments. The role supports a non-prescribing model focused on health education, chronic condition management, and coordination with primary care providers to ensure seamless patient care. The position offers a unique work-life balance with flexible scheduling between 8 am and 8 pm over a seven-day week, excluding weekends, holidays, and on-call duties, supporting providers in meeting personal and professional needs.

As an APC with Optum Home & Community Care, you will travel extensively across the designated states, approximately 85% of the time, with all travel expenses covered. You have the freedom to live anywhere within your state of practice while contributing to impactful healthcare delivery. Compensation for this role ranges between $109,500 and $164,000 annually, comprised of base salary plus incentives, and includes a $30,000 sign-on bonus or up to $40,000 student loan repayment for eligible external and internal candidates.
